摘　　要：目的 :进一步探讨神经性关节病的X线征象和治疗。方法 :回顾性分析 2 0例神经性关节病 2 3个关节的X线征象 ;手术治疗 16例 ,其中肘关节置换术 4例。结果 :2 1个关节出现骨膜反应 ,其中 12个骨膜密度低于骨皮质并与骨干间有透亮线 ,9个骨膜密度等于骨皮质并与骨干融合 ,前者骨质吸收程度较后者重。 15个关节的钙化位于关节间隙及其附近 ,数量多、体积小且不规则 ,11个关节的钙化或骨化距关节腔较远 ,数量少、体积大、边缘光滑。 4例肘关节置换术获得 9～ 2 2个月随访 ,优 2例 ,良 2例。结论 :骨膜密度及形态能反映本病的进展程度 ,不同部位的异位钙化或骨化其形态各有特点。在一定条件下 ,关节置换术可以应用于神经性关节病的治疗。Objective: To discuss the X ray features and the treatment of neurarthropathy.Methods:The clinical manifestations and imaging features of 20 patients were reviewed.16 cases were underwent operation,among them 4 cases were elbow replacement arthroplasty.Results:Periosteal thickening are found in 21 joints,12 of which are lower density than the diaphyseal cortex and have bright space to the bone stem,9 are higher density than the diaphyseal cortex and stick to the bone stem,the bone resorption is haveier in the former.The 15 heterotopic ossicula near the joint are small,in large quantity and various in shape,the 11 heterotopic ossicula far from the joint are big,in small number and smooth edged.4 cases underwent elbow replacement were followed up from 9 to 22 months,2 were excellent and 2 were good.Conclusions:The density and the shape of the periosteal thickening could reflect the extent of the neurarthropathy,the shape of heterotopic ossicula are related to its location.Under certain circumstances,joint replacement could be used to treat neurarthropathy.
