摘 要:目的研究肝组织中转化生长因子β1(TGF-β1)的表达与慢性乙型肝炎(CHB)肝血管增生及肝血管纤维化的关系方法 CHB标本120例,CCl_4实验鼠肝标本50例进行TGF-β1免疫组织化学(免疫组化)染色及TGF-β1 mRNA原位杂交,并检测了CHB肝组织中α-SMA,FN,LN,Co-Ⅳ表达与TGF-β1的关系结果在CHB Ⅰ组TGF-β1阳性表达占93%,强阳性表达为70%;在CHB Ⅱ组其阳性表达占43%,强阳性表达为57%;在CHB Ⅲ组其强阳性表达占77%,阳性表达为23%;而在肝硬变(LC)组TGF-β1则皆示强阳性表达(100%),其间有极显著性差异(P<0.01)原位杂交显示,TGF-β1mRNA主要分布于血管纤维化区域、肝窦壁及部分肝细胞,同时TGF-β1在肝组织中的表达与α-SMA,FN,LN及Co-Ⅳ呈同步关系结论TGF-β1通过激活血管及肝窦内皮细胞,引发肝血管增生及内皮细胞凋亡,在细胞外基质等参与下,最终导致肝血管纤维化、肝窦毛细血管化及假小叶纤维间隔形成.AIM To study the relationship between the expression of transforming growth factor beta-1 (TGF-β1) and hepatic vascular proliferation and fibrosis in chronic hepatitis B (CHB). METHODS Immunohistochemical staining (TGF-β1) and in situ hybridization (TGF-β1 mRNA) were performed on hepatic tissues of 120 cases of CHB and 50 cases of rats induced by CCl_4. The relationship between the expression of α-SMA, FN, LN, Co-Ⅳ and TGF-β1 were also determined. RESULTS In CHB Ⅰgroup, the positive expression of TGF-β1 occupied 93%, and the strongly positive expression 7%; in CHB Ⅱ group, the positive expression was 43%, the strongly positive expression was 57%; in CHB Ⅲ group, the strongly positive expression was 77%, and the positive expression was 23%; while in LC group, the expression was all strongly positive (100%), there were significant differences between them (P<0.01). The result of in situ hybridization showed that TGF-β1 mRNA mainly distributed on area with vascular fibrosis, hepatic sinusoidal walls and some hepatocytes. They were synchronous of the expression of TGF-β1 and α-SMA, FN, LN and Co-Ⅳ. CONCLUSION TGF-β1 can initiate the hepatic vascular proliferation and the apoptosis of endothelial cells through activating vessels and hepatic sinusoidal endothelial cells. And finally induces the formation of hepatic vascular fibrosis, hepatic sinusoidal capillarization and fibrous septum of pseudolobuli with the participation of extracellular matrix.
