甲基强的松龙冲击疗法治疗病情反复严重的原田综合征

摘　　要：目的　评价甲基强的松龙冲击疗法 (简称甲冲法 )治疗病情反复严重的原田综合征的疗效。方法　从病情反复的严重原田综合征中选择一般疗法无效 ,又无肾上腺皮质激素禁忌的病例 2 0例 (4 0眼 )为研究对象 ,甲冲法是用甲基强的松龙 5 0 0～ 10 0 0 mg加入 5 %葡萄糖溶液 5 0 0 ml静脉滴注 ,每天 1次 ,3～ 5天为一疗程。以后再用一般疗法。结果　显效 (视力提高 6排以上 ) 14例 (2 8眼 ) ,占 70 %眼 ;有效 (视力提高 3～ 5排 ) 4例 (8眼 ) ,占 2 0 %眼 ;总有效率为 90 %。无效 (视力 <0 .1) 2例 (4眼 ) ,占 10 %眼。未发现副作用。随访 10个月～ 1年无复发。结论　甲冲法治疗病情反复严重的原田综合征 ,作用迅速可靠 ,疗效高 ,未发现副作用 ,随访无复发。Objective To estimate the curative effects of methylprednisolone pulse therapy(MPPT)on the recurrent serious harada's disease.Methods 20 cases(40 eyes)with the recurrent serious harada's disease,which failed to respond to general treatment and no contraindication with steroid,were treated with MPPT.MPPT comprises large doses of methylprednisolone(0.5～1.0g),dissolved in 500ml of 5% glucose solution,given intravenously daily for 3 to 5 days,followed by conventional therapy.Results Obviously effective(vision acuity increase 6 rows and above):14 cases(28 eyes)takes 70%;effective(vision acuity increase 3～5 rows):4 cases (8 eyes)takes 20%,total effective rate takes 90%;No effect(vision acuity<0.1):2 cases(4 eyes) takes 10%.No side-effect was observed and no recurrence after follow-up ranged from 10 months to 12 months.Conclusions The MPPT is a reliable methods in treatment of the recurrent harada's disease,no side-effect and no recurrence was observed.
