摘 要:目的 探讨相对淋巴细胞浓度 ,即应激反应程度对判断急性心肌梗塞病人预后的价值。方法 对 2 6 9例被诊为AMI的入院患者 ,排除其它引起慢性应激的因素 (38例 ) ,共有 2 31例患者进入了研究组。其中男性 15 7例 ,女性 74例。平均年龄 (6 3 .3± 11.0 )岁。于入院后 2 4~ 48小时内用自动分析仪进行血球计数分析 ,观察住院生存情况。结果 相对淋巴细胞计数正常组 (I组 ,淋巴细胞的相对浓度≥ 2 0 % )共有 6人死亡 ,死亡率为 4.8% (3/ 6 2 ) ;相对淋巴细胞计数降低组 (II组 ,淋巴细胞的相对浓度 <2 0 % )共有 19人死亡 ,死亡率为 17.9% (19/ 10 5 )。两组差异有显著性 (χ2 =10 .2 37,P =0 .0 0 1)。结论 相对淋巴细胞计数作为应激水平的一个标志 ,是AMI判断近期预后的重要指标之一。Objective The purpose of the present study is to investigate whether the relative lymphocyte concentration, which represent severity of stress, could predicts in hospital survival of patients with acute myocardial infarction (AMI). Methods A total of 269 consecutive admitted patients diagnosed as AMI were studied. Blood count was performed in 24 48 hours after admission and echocardiography was performed in third week after admission; Of 269 cases, 38 were excluded because of chronic stress. Survival or death was observed when discharged. Results There were 6 patients died in normal relative lymphocyte concentration group (Group I, relative lymphocyte concentration ≥ 20%) during hospitalization, and the death rate was 4.8% (6/124). There were 19 patients died in decreased relative lymphocyte concentration group (Group II, relative lymphocyte concentration < 20%), and the death rate was 17.9% (19/105). The difference of death rate between the two groups was significant (χ 2 = 10.237 P=0.001) Conclusion The relative lymphocyte count, as a maker of stress response, can predicts survival of AMI patients during hospitalization. Measurement should be taken to reduce the physical and psychological stress.
