高血压合并高脂血症患者血管内皮依赖性舒张功能的损害

摘　　要：目的 :探讨高血压合并高脂血症患者血管内皮依赖性舒张功能的变化。方法 :采用无创伤性高分辨超声技术检测单纯高血压 (n=14)、高血压合并高胆固醇组 (n=2 4) ,和健康人 (n=2 0 )休息时 ,反应性充血 ,舌下含服硝酸甘油(GTN)后肱动脉内径变化。结果 :发现单纯高血压组和高血压合并高胆固醇组肱动脉血流介导性扩张百分比明显低于正常组 (P<0 .0 5 ) ,尤以合并高胆固醇组患者下降更明显 ,高血压组之间的差异亦有显著性 (P<0 .0 5 ) ;舌下含服GTN后肱动脉内径变化在单纯高血压组与正常对照组相比无显著性差异 (P>0 .0 5 ) ,但高血压合并高胆固醇组与正常对照组相比有显著性差异 (P<0 .0 5 )。结论 :原发性高血压存在血管内皮依赖性舒张功能失调 ,合并高胆固醇血症者尤为明显 。Objective : To detect the changes of endothelium dependent vascular relaxing fu nction in the hypertensive patients with hypercholesteolemia . Methods : With high resolution ultrasound , the diameter of brachial arteries at rest , during reactive hyperaemia and after sublingual GTN were measured in 58 subjects , consisted of 14 cases of essential hypertension , 24 cases of essential hypertension with hypercholesterolemia and 20 normal controls . Results : The results showed that the flow mediated dilatation (FMD) of brachial artery in the groups of essential hypertension and essential hypertension with hypercholesterolemia was significantly reduced in comparison with the normal control group( P <0.05), especially in the complicated with hypercholesterolemia.However , there was no significant difference in response to GTN between the essential hypertension and the control group( P >0.05); but there was significant difference in response to GTN between the essential hy pertension with hypercholesterolemia and the control group( P <0.05) . Conclu sions : Endothelium dependent vasolidation is impaired in essential hyperten sion patients , and more severely in essential hypertension with hypercholester olemia. The results suggest that hypercholesterolemia may gradually imp air endothelium dependent vasodilation.
