摘　　要：目的 :与血管造影对比 ,评价三维能量多普勒超声在观察肿瘤血管上的可靠性和对肿瘤定性诊断的准确性。方法 :5 8例经病理证实的腹部肿瘤患者接受三维能量多普勒超声检查 ,其中肝细胞性肝癌 2 5例 ,肾癌 5例 ,肝转移癌 16例 ,肝血管瘤 8例 ,肾血管瘤 4例。 5 8例中 2 8例在一周内进行血管造影。结果 :三维多普勒超声所显示的肿瘤血管图像与血管造影的动脉期图像非常相似。在原发性肝癌和肾癌病例 ,三维能量多普勒超声显示出肿瘤周边粗大、弯曲的供血动脉向肿瘤内部延伸。在肝转移瘤病例 ,可见肿瘤周边正常粗细的血管受压变形、移位并环绕肿瘤。在肝血管瘤 ,表现为肿瘤内部无明显血管 ,肿瘤周围可见部分血管环绕。结论 :肿瘤三维能量多普勒超声图像与血管造影动脉期图像非常相似 ,能准确显示肿瘤内部及周边的血流情况 ;三维能量多普勒超声显示肿瘤血流的明显差异有助于鉴别肿瘤的性质。Objective:To evaluate the value of three dimensional(3D)power Doppler sonography demonstrating tumor vascularity correlating with angiography.Methods:3D power Doppler sonographic examinations were performed in 58 cases with pathologically confirmed tumors including 25 cases with hepatocellular carcinoma(HCC),renal carcinoma 5 cases,hepatic metastasis 16 cases,hepatic hemangioma 8 cases,and renal hemangioma 4 cases.Twenty eight of the 58 patients underwent angiography within one week.Results:The images of 3D power Doppler sonography and angiography(arterial phase)were quite similar in demonstrating tumoral vascularity.In HCC and renal cell carcinoma,the tortuous and bulky blood vessels extending into the tumors from their surrounding regions were visualized by 3D power Doppler sonography.In the cases with hepatic metastasis,3D power Doppler sonography showed the compressed,distorted normal sized vessels surrounding the tumor.In the cases with hepatic hemangioma,no obvious vessels were found in the tumor and only some vessels were detected around the lesion.Conclusions:3D power Doppler sonography resembles angiography in arterial phases in which it can demonstrate correctly the intratumoral and peripheral vasculatures and their blood flows.The distinct discrepancies of their blood flows are helpful for the differentiation between the characteristics of tumors.
