慢性阻塞性肺病嗜麦芽窄食单胞菌感染24例临床分析

摘　　要：目的　分析慢性阻塞性肺病嗜麦芽窄食单胞菌感染的临床特点及药物敏感性。方法　对 2 4例嗜麦芽窄食单胞菌感染的慢性阻塞性肺病患者的临床资料进行统计分析 ,用微量稀释法进行药物敏感性测定。结果　2 4例全部接受广谱抗生素治疗 ,5 4%接受静脉激素治疗 ,5 0 %合并其他慢性疾病 ,2 5 %曾接受机械通气。临床表现缺乏特异性。药敏试验提示该菌对复方磺胺甲唑 10 0 %敏感 ,替卡西林 /克拉维酸 5 4%敏感 ,其余药物均低于 5 0 %。结论　嗜麦芽窄食单胞菌感染多发生在长期使用广谱抗生素 ,免疫功能下降的患者。临床表现类似一般革兰氏阴性菌所致感染 ,该菌广泛耐药 。Objective To analyze the clinical characteristics of chronic obstructive pulmonary disease infected by Xanthomonas maltophilia and the drug sensitivity of Xanthomonas maltophilia strains.Methods Retrospective study of the clinical data of 24 cases with chronic obstructive pulmonary disease infected by Xanthomonas maltophilia.Drug sensitivity against strains of Xanthomonas maltophilia was studied by micro dilution method.Results All cases were given broad-spectrum antibiotics,54% of the cases were given intravenous treatment with steroids,50% of the cases were complicated in other chronic diseases,25% of the cases were given mechanical ventilation.Clinical manifestations were lack of specificity.The drug sensitivity test showed that 100% of these strains of Xanthomonas maltophilia were susceptible to SMZco,54% were susceptible to ticarcillin/clavulanic acid and the sensitive rate of other drugs was under 50%. Conclusion The chronic obstructive pulmonary disease infected by Xanthomonas maltophilia mostly occurred in the patients treated by broad-spectrum antibiotics for long period and in the immunocompromised patients.Clinical manifestations were similar to those of infection caused by general gram-negative bacteria.Xanthomonas maltophilia was widely resistant.It is the key for diagnosis and treatment that the results of bacteria culture and drug sensitivity should be obtained in time.
