地氟醚吸入麻醉时呼吸回路内药物浓度分析

摘　　要：目的 :探讨地氟醚吸入麻醉时呼吸回路内药物浓度变化过程及年龄因素对回路内药物浓度的影响。方法 :拟行全身麻醉、年龄≥ 6 5岁的 30例患者为老年组 ,2 8～ 45岁 30例患者为中青年组。静脉诱导后气管插管 ,接麻醉机行间歇正压通气 ,保持氧流量 1L/min。开启地氟醚定量挥发器到 8Vol% ,30min后关闭挥发器 ,开启吸气回路内麻醉药吸附器继续通气 1 0min。结果 :持续吸入地氟醚 30min时老年组和中青年组平均CI 分别为 6 96 %和 7 1 5 % (P <0 0 1 ) ,CE 为 6 0 8%和 6 43% (P <0 0 1 ) ,CE/CI 为 0 87和 0 90 (P <0 0 1 ) ,两组CI 与CE 与地氟醚吸入时间的常用对数呈高度相关 ,CE/CI 的倒数与地氟醚吸入时间的倒数呈高度相关。停止吸入地氟醚后仅 44 7s (老年组 )和 5 1 8s (中青年组 )CE 降到 3 6Vol% (P >0 0 5 )。结论 :成年患者固定载气流量吸入恒定输出浓度地氟醚 ,回路内麻醉药浓度与吸入时间高度相关 ,可按相应回归方程计算出 30min内地氟醚浓度。相同吸入时间老年患者CI、CE 和CE/CI 比值均低于中青年患者。Objective:To study the changes of desflurane concentration and to analyses the concentration effected by age Methods:60 patients was divided into two groups,one was older than 65 yrs,the other was 28 to 45 yrs old Endotracheal intubation and IPPV were performed after intravenous induced The oxygen flow was maintained in 1 L/min steadyly The desflurane vaporizer was opened at 8%,and closed after 30 min Then,the anesthetic absorber placed in inspired airway was opened immediately Results:The average C I of the older group and young group was 6 96% and 7 15% in the 30 th min,respectively( P <0 01);C E was 6 08% and 6 43%,respectively( P <0 01);C E/C I was 0 87 and 0 90,respectively( P <0 01) The C E and C I of desflurane in both groups were highly correlative to logarithm of inhalation time The reciprocal of C E/C I was highly correlative to that of reciprocal of inhalation time After discontinued inhaled desflurane,it took only 44 7 s(older group)and 51 8 s(young group)for C E to dropped to 3 6 Vol%(0 6 MAC)( P >0 05) Conclusion:When the output of desflurane vaporizer and carried air flow are constant,C I and C E of the circuit are highly correlative to the inhalation time in the adult The desflurane concentration within 30 min can be calculated with regressive equation The C E/C I,C I and C E of olders are lower than those of the young patients at the same time
