摘 要:[目的 ]探讨最佳的促超排卵方案 ,以期获得高质量的卵细胞 ,并尽可能减少卵巢过度刺激综合征 (OHSS)的发生。[方法 ]应用GnRH a长、短方案促超排卵 ,比较受精、卵裂、妊娠率及OHSS的发生率。在长方案中比较Buserelin和达必佳降调节及对卵巢的抑制效果。[结果 ]GnRH a长方案及短方案在卵子回收率、受精率、卵裂率及妊娠率方面差异无显著性 ,OHSS发生率以GnRH a长方案组较低。长方案中达必佳小剂量 (0 0 5mg/日 )皮下注射与Buserelin比较 ,两组FSH、LH的下降率相似而Gn的用量差异无显著性 ,Buserelin组中有 2例LH峰提前出现而达必佳组未出现。 [结论 ]GnRH a长方案可获得较为满意的受精率、卵裂率、妊娠率 ,OHSS的发生率明显低于短方案 ,小剂量达必佳可获得较Buserelin更为满意的抑制效果 。Objective] Searching for the optimal schemes of ovarian hyper stimulation in IVF ET cycles to get high quality oocytes and decrease the incidence of ovarian hyper stimulation syndrome (OHSS) [Method] Two treatment schemes (long and short time) of GnRH a for controlled ovarian hyper stimulation (COH) were compared in the rate of fertilization, cleavage and pregnancy, the incidence of OHSS In long time scheme, the regulation effects of ovarian function were compared between Buserelin and Decapeptyl [Results] The rates of oocytes retrieve, fertilization, cleavage and pregnancy were similar for two schemes The incidence of OHSS in short time scheme was higher than in long one Compared the mini dose decapeptyl (0 05 mg daily) with the Buserelin, the decreasing rate of FSH and LH and the Gn dosage were similar but in the Buserelin, the peak of LH appeared in advance [Conclusion] The long and short protocols of GnRH a can get similar fertilization, cleavage and pregnancy rates but the OHSS rate in long scheme was lower than in short one Mini dose Decapeptyl can achieve more satisfactory down regulation effect than Buserelin without ovarian over suppression
