带旋前方肌桡骨膜瓣移植治疗腕舟骨骨不连

摘　　要：目的　总结采用带旋前方肌桡骨膜瓣移植治疗腕舟骨骨不连的方法和疗效。方法　于腕桡掌侧切取带旋前方肌的近端桡骨膜 5cm× 2cm ,将桡骨膜翻转包裹适宜的桡骨块植入舟骨骨槽内。术后管型石膏固定 3个月。结果　治疗腕舟骨骨不连 2 0例 ,术后随访 8个月～ 6年 ,平均 1 8年 ,根据刘树清等的功能评定标准 ,结果优 18例 ,良 2例。愈合时间 2～ 4个月 ,腕关节功能完全恢复正常。结论　旋前方肌桡骨膜瓣有为受骨提供丰富血运和成骨的双重功能 ,有效地促进了骨折愈合 ,且该手术操作简便 。Objective To describe the method and effectiveness of using flap of radial periosteum with muscle pronator quadratus grafting on carpal scaphold nonunion.Methods A 5cm×2cm proximal radius periosteum with muscle pronator quadratus pedicle was harvested in the palm of the hand and then the periosteum was turned over and packed the suitable proximal radius into the space.The hand was immobilized in a cast plaster for 3 months after operation.Results 20 cases of carpal scaphoid nonunion were treated and followed up for an average of 1.8 years(8 months to 6 years).Based on the function evaluation standard of LIU Shu qing′s,18 cases had excellent and 2 good results.All 20 cases had bone union 2 to 4 months after surgery and complete restoration of wrist function was achieved.Conclusion Flap of radial periosteum with muscle pronator quadratus has double functions of supplying blood circulation to carpal scaphoid and osteogeresis.This method was effectiveness,simpleness and should be recommended for wide use.
