伊贝沙坦对原发性高血压患者心率变异性的影响

摘　　要：目的：研究血管紧张素１１受体拮抗剂伊贝沙坦（ｉｒｂｅｓａｒｔａｎ）对原发性高血压患者心率变异性（ｈｅａｒｔ ｒａｔｅ ｖａｒｉａｂｉｌｉｔｙ， ＨＲＶ）的影响。方法： ２９例原发性轻中度高血压患者，服伊贝沙坦（１５０－３００ｍｇ·ｄ－1，服８周，ｎ＝２９）前后行２４ｈ动态心电图检查，分析心率功率谱时域和频域指标。结果伊贝沙坦使ＨＲＶ时域指标ＳＤＮＮ、ＳＤＡＮＮ、ｒＭＳＳＤ、ＳＤＮＮＩ和频域指标ＨＦ轻度增加，频域指标ＴＦ、ＬＦ和ＬＦＩＨＦ明显减低，与治疗前比，频域指标有显著性差异。伊贝沙坦使收缩压（ＳＢＰ）和舒张压（ＤＢＰ）显著降低。结论：伊贝沙坦对原发性高血压患者降压的同时，对心率变异性产生有益的作用。OBJECTIVE: To evaluate the effects of irbesartan on heart rate variability (HRV) .METHODS: HRV were observed using 24-hour ambulatory ECG before and after taking the irbesartan, in 29 patients with mild-to-moderate essential hypertension, RESULTS: The angiotensin recepter antagonist(irbesartan 150300mg once daily for & weeks n=29) reduced blood pressure(from 152if02 to 132/ 89mmHg) together with increase of the standard deviation of normal RR intervals (SDNN), standard deviation of 5-minute mean RR intervals (SDANN),the square root of the mean of squared difference between adjacent normal RR intervals (rMSSD) and high frequency(HF) components of power spectrum density, but total frequency (TF),low frequency(LF) and high/low frequency ratio were markedly reduced. CONCLUSION: It implies irbesartan therapy can significantly increase the heart rate veriability of patients with essential hypertension.
