高血压患者胆固醇甘油三酯与脑卒中发病率关系4年随访

摘　　要：目的 :观察高血压患者血清胆固醇、甘油三酯水平 ,力图探讨其与脑卒中发病率之间的关系 ,为高血压患者预防脑卒中提供治疗参考。方法 :选择无脑卒中病史的高血压患者进入本观察。 187例完成随访。每年检查 1次血清胆固醇、甘油三酯。每个患者至少行 1次头颅CT检查。患者出现脑卒中或死亡为本观察中止指标。随访采用专科门诊与住院治疗相结合的办法。观察结束将卒中组与非卒中组进行对比分析。结果 :187例患者中 2 9例检出脑卒中 (15 5 1% ) ,12例确诊为脑出血。卒中组平均血压为 176 90 10 0 17mmHg(非卒中组 173 6 3 10 2 6 0 ,P =0 5 4,0 44 )。胆固醇 5 0 4mmol L(Vs 5 0 9mmol L ,P =0 87) ,甘油三酯为 2 44mmol L(Vs 1 5 2mmol L ,P =0 0 2 5 )。结论 :本组观察对象均为高血压患者 ,卒中发病率较高 ,2组血清胆固醇值无明显差异。甘油三酯卒中组明显高于非卒中组。本组观察提示 :高血压患者预防脑卒中关键是降压 ,至于调脂治疗 ,似以降低甘油三酯为主 ,但由于观察例数、观察时限的局限 。Objective:By observing the serum levels of CHO,TG of hypertensive patients,the relationship between these levels and the occurrence rate of stroke in those patients were discussed.We hope we can give hypertensive patients referential treatment to avoid stroke among them.Method:Hypertensive patients without stroke were chosed to be observed and 187 cases had been finished.All of them had been examined the levels of CHO,TG,and performed CT scan at least once a year when they died or stroke oucured,we stopped the observation.The follow up combined the treatment of inpatient patients and outpatient ones,and analysed them by comparing to those who occurred stroke with those not.Result:29 cases were found stroke in 187 cases (15 51%),12 cases were diagnosed as hemorrhage stroke.The average level of blood pressure of the stroke was 176 90/100 17 mmHg(that of patients without stroke was 173 63/102 60 mmHg, P= 0 54,0 44)CHO 5 04 mmol/L (Vs 5 109 mmol/L, P =0 87),TG2,44 mmol/L(vs 1 52 mmol/L, P =0 0025).Conclusion:The targets being observed were all hypertensive patients,the rate of stroke occurrence among them was high.The levels of CHO in both groups had no signifcant difference,but the levels of TG of thoes who had stroke were significantly than those without.It suggested that in order to prevent stroke for hypertensive patients,the key method is to reduce the levels of blood presscure,as for blood fat,it seemed that the most important treatment is to reduce the levels of TG.For the numbers of the cases and the duration of observation are limited,well further the discussion afterwards.
