Comparative Results of Retrograde Intramedullary Interlocking Nail and Compressive Plate in Treating Distal Fracture of Femur

摘　　要：目的 :观察逆行交锁髓内钉对治疗股骨中下段骨折的疗效。 方法 :对 34例股骨中下段骨折采用逆行交锁髓内钉治疗 ,并进行随访 ,与 34例采用加压钢板治疗股骨中下段骨折疗效进行对比分析。 结果 :34例均痊愈 ,无断钉、感染、再骨折发生 ,功能恢复正常。 结论 :逆行交锁髓内钉通过纵向与髓腔内紧密贴合固定控制骨折断端旋转并对断端进行轴向加压 ,固定可靠 ,便于膝关节早期功能锻炼 ,同时最大限度地保护了骨折端的血运 。To prevent and to treat the rotation,displacement,unstability,and difficulty for fixation in fracture of distal femur,the clinical outcome of retrograde intramedullary interlocking nail is assessed Methods:Thirty-four cases of fracture of distal femur were treated by retrograde intramedullary interloching nail.The results were compared with another 34 cases fixed by compressive plates.Results:All fractures had a clinical union in those fixed by retrograde intramedullary interlocking nail.The healing time was 5～12 months with an average of 7 months.All patients resumed good knee function,and with no infection.Complications in the compressive plate group include 2 plate broken,2 screw broken and 1 refracturing.Conclusion:The retrograde intramedullary interlocking nail therapy can cntrol the rotation of the fractured shaft exert axial compression force to the bone frame through medullary fixation,providing a stable and reliable fixation of the fractured ends,beneficial to early functional exercise.The blood supply of the fractured ends have been protected,thus provides a good biologic environment for healing.
