颈椎颈髓损伤早期手术治疗疗效评价(附37例报告)

摘　　要：目的 :探讨早期手术对颈脊髓神经功能恢复方面的作用。方法 :通过脊髓神经功能评价方法 ,分析总结一组伤后 3d内实施手术减压和 /或固定的颈椎脊髓损伤患者的预后。结果 :术后第 15d ,2 3例脊髓不完全性损伤者运动和感觉神经功能分别改善 (ASIA神经功能评分法 ) 9.12和 9.30分 ,相差显著 ,14例脊髓完全性损伤者运动和感觉神经功能分别改善 0 .43和 1.43分 ,有改善。术后 12个月随访 ,9例脊髓不完全性损伤者运动和感觉神经功能分别改善 4.0 0和 18.33分 ,相差显著 ,6例脊髓完全性损伤者运动和感觉神经功能分别改善 0 .5 0和 1.0 0分 ,无显著改善。结论 :伤后3d内实施手术脊髓减压和颈椎内固定术 。Objective:To evaluate the result of early operation for the treatment of cervical spinal cord injury.Methods:Motor score and sensory score were analyzed on 37 cervical spinal cord injury patients who underwent operation with in 3 days after injury.Patient was evaluated at admission,before discharge and at 12 months after operation.Results:15 days after operation,the motor and sensory function improvement were very significant in 23 cases incomplete cervical spinal cord injury patients and were significant in 14 cases complete cervical spinal cord injury.12 months follow up,the motor and sensory function improvement were significant in incomplete cervical spinal cord injury patients and were not significant in complete cervical spinal cord injury.Conclusion:Early operation for the treatment of cervical spinal cord injury significantly improve the motor and sensory function of the cervical spinal cord injury patients.
