摘　　要：目的探讨同种原位背驮式肝移植术(PBOLT)常见手术并发症的预防。方法4例终末期良性肝病行5次 PBOLT,其中2例为肝肾联合移植,1例为肝移植术后慢性排斥行再次肝移植。结果5次肝移植术后肝功能恢复良好,未发生与背驮式手术相关的并发症,最长存活1年,生活质量良好。结论 PBOLT 可适用于任何终末期良性肝病患者,包括再次肝移植。正确处理第三肝门,以三根肝静脉成形后与供肝肝上下腔静脉吻合,术中维持血液动力学稳定是减少或避免该术式有关手术并发症的重要措施。Objective To investigate prevention of the common complications of piggy-back orthotopic liv- er transplantation(PBOLT).Methods Four cases(5 times)of PBOLT were performed for the benign end-stage liver diseases.Two cases among them were combined transplantation of liver and kidney,one of which was liver retransplantation for the transplanted liver because of chronic rejection.Rusults The liver function restored satis- factorily postoperation in all patients.No complications relaed to the piggy-back technique were found.The longest survival was one year and got good life quality war adieved in the patient.Conclusion The piggy-back technique can be used routinely in all patients with benign end-stage liver diseases and facilitate retransplantation,and one recults in few surgical complications.It is important for preventing postoperative complications to take care of man- agement of the third porta hepatis,use the patch obtained by joining the three hepatic veins for the anastomosis and maintain stable hemodynamics during operation.
