HOE-642联合加镁钾停搏液(K/Mg)对成年心肌的保护作用

摘　　要：目的　探讨Na+ /H+ 交换蛋白抑制剂 (NHE I)作用的最佳时段以及HOE 6 42联合加镁钾停搏液 (K/Mg)对成年心肌缺血 再灌注 (I R)损伤的保护效果。方法　成年兔心Langendorff灌注 ,分 :①单纯K H灌注 ;②单纯GI;③K/Mg ;④K/Mg I,HOE I;⑤K/Mg I,HOE R ;⑥K/Mg I,HOE IR共六组 ,测定比较各组心功能、心肌梗死量的变化。结果　所有HOE 6 42联合K/Mg组 (组④～组⑥ )心肌梗死量均明显减低 ,以组⑥最小 ,分别为 (4 .6± 0 .7) %、(4 .7± 0 .9) %、(1.3±0 .4) % (与组①、组②、组③比较 ,P <0 .0 5 ) ,左心功能恢复亦明显改善。结论　HOE 6 42联合K/Mg能减轻心肌I R损伤 ,促进心功能恢复 ,并以I前和R早期同时使用效果最佳。提示在这两个时段均可抑制NHE 。Objective To investigate when is the optimal target time of inhibiting the activity of Na +/H + exchange inhibitor (NHE I) and whether the combination of NHE I (HOE 642) with magnesium supplemented potassium cardioplegia (K/Mg) confers additional cardioprotective effects against ischemia reperfusion ( I R) injury. Methods Thirty six mature rabbit hearts were randomly divided into six groups ( n =6 in each group), perfusing with K H solution. Hearts were perfused throughout the experiment without I R (group 1); subjected to I R without K/Mg (group 2); subjected to I R and received K/Mg (group 3) or K/Mg plus HOE 642 (group 4 through group 6); Infarct size, left ventricular function and coronary flow was evaluated in all groups. Results IS was markedly reduced in HOE 642 plus K/Mg treating groups, i.e., to( 4.6 ± 0.7 )%, ( 4.7±0.9 )%, (1.3±0.4)% in group 4, group 5 and group 6, respectively ( P <0.05 versus group 1, group 2 and group 3). The postischemic functional recovery was also significantly improved in these groups. Conclusion Treatment with HOE 642 plus K/Mg may attenuate I R injury and thereby enhance the recovery of post ischemic left ventricular function. Furthermore, the maximal cardioprotective effects could be obtained when HOE 642 was administered during early R as well as before I, suggesting inhibition of NHE I occurs in both of these courses.
