摘  要:目的 应用自体肌基膜管 (musclebasallamina ,MBL)移植结合神经生长因子 (nervegrowthfactor,NGF)修复脊髓缺损。 方法  18只雌性家犬脊髓横断后分为三组。A组 :MBL移植结合注射NGF组 ,7只犬 ;B组 :单纯MBL移植组 ,7只犬 ;C组 :对照组 ,切除脊髓 0 .5cm ,4只犬。 6个月后用免疫组化法对神经轴突进行特异染色 ,并用图像分析方法对脊髓横断处的远近端横截面进行神经纤维数比较。 结果 A、B两组移植物中神经纤维计数比较 ,差异有非常显著性意义(P <0 .0 1) ;各组远端神经纤维数比较 ,差异有非常显著性意义 (P <0 .0 1) ;近端神经纤维数比较 ,A组与B组差异有显著性意义 (P <0 .0 5 ) ,A组与C组差异有非常显著性意义 (P <0 .0 1) ,而B组与C组差异无显著性意义 (P >0 .0 5 )。光镜下观察到MBL移植物中有神经纤维通过。电镜证实横断远端存在新生神经纤维。 结论 MBL作为一种生物管道 ,可作为神经轴突再生的通路 ;NGF具有促进神经轴突生长的作用。MBL移植结合NGF修复脊髓横断性损伤 ,神经轴突能越过移植区到达远端。Objective To observe the histological changes of transp lanted autologous muscle basal lamina (MBL) and using nerve growth factor (NGF) to repair spinal cord transection in the adult dogs. Methods Laminectomy was done in 18 adult female dogs over T 13 and L 1. The animal s were d ivided into 3 groups. In Group A: 7 dogs, a gap of 1 cm was done longitudinally on the spinal cord at the lesion site. Autologous MBL grafts made from paraspin al muscle fibers were implanted to the spinal cord gaps. NGF was administered at the lesion site via dwelling implanted osmotic minipumps. In Group B: 7 dogs, a gap of 1 cm was done longitudinally on the spinal cord at the lesion site. Auto logous MBL grafts made from paraspinal muscle fibers were implanted to the spina l cord gaps alone. In Group C: 4 dogs served as control and each animal received a complete transection on the spinal cord. Neurological function was observed f or 6 months and the animals were sacrificed. The grafted materials and the treat ed spinal cord were procured for histological examination. The characteristics of regeneration were observed histologically by immunocytochemistry (ICC) method and electron microscope. All the immunostaining pictur es were analyzed by the method of morphometric analysis which gave out the data of the axons counting. Results The results showed significantly be tter microscopic and electron microscopic appearance in Group A with implantatio n of both MBL and NGF. Nerve fibers were identified in the graft by immunohistoc ytochemical staining for NGF. Degeneration and necrosis were found in Group C. L i ttle degeneration and necrosis occurred in Group B and Group A. Conclu sions The data show that MBL can provide a suitable environment for axo nal regeneration and NGF is able to enhance the regeneration capacity of central nervous system.
