氯沙坦和氨氯地平对原发性高血压肾损害患者的疗效比较

摘　　要：目的 :比较氯沙坦与氨氯地平对原发性高血压 (EH)肾损害患者的降压效果 ,并评价药物对肾功能的影响。方法 :将 5 8例属于缓进型Ⅰ期～Ⅱ期原发性高血压患者分为氯沙坦组 (2 9例 )和氨氯地平组 (2 9例 ) ,观察给予氯沙坦 5 0mg ,每日 1次口服或氨氯地平 5mg ,每日 1次口服后 2、4、6、8周的坐位血压、心率及治疗前后内生肌酐清除率 (Ccr)、尿微量白蛋白 (Alb) ,计算其排泄率 (UMAE)及测定血尿 β2 微球蛋白 (β2 MG)和N 乙酰 β D 氨基葡萄糖苷酶 (NAG)的变化。结果 :两药治疗后收缩压和舒张压各次测值分别较基值降低 ,有显著差异性 (P <0 0 5 )。氯沙坦组降压幅度略大于氨氯地平组 ,氯沙坦组治疗前后Ccr有显著差异性 (P <0 0 5 ) ,UMAE有显著差异性 (P <0 0 1) ,氨氯地平组治疗前后Ccr有显著差异性 ,UMAE无显著差异性。结论Objective To compare antihypertensive efficacy of lo sartan and Amlodipint in patients of primary hypertension with renal damage and estimate the effect of drugs on renal function.Method 58 patients with Ⅰ-Ⅱ stage benign primary hypertension were divided into losartain admin istrating group (29 cases) and Amlodipint administrating group (29 cases).After treated with losartan 50mg once a day or Amlodipint 5mg once a day for 2.4.6.8. weeks,observations of the sitting position blood pressure;heart rate;endogenous creatinine clearance rate (CCR) before and after treatment;urinary microalbumin (Alb);excretion rate (UMAE) and β2-microglobulin (β2-MG) and N-acetyl-β- D-glucosamidase (NAG) in blood and urine were made.Results After taking these two drugs,every measured titers of systolic pressure and diastolic pressure of the patients are respectively lower than basic titer and have obvio us difference (P<0 05).The declining amplitude of losar tan group is a bit bigger than that of Amlodipint group.The CCR and UMAE of losa rtan group have obvious difference before and after treatment (P<0 01).As to Amlodipint group,the CCR does,but the UMAE doesn't.Conclusion Losortan is much better than Amlodipint in the aspect of ant ihypertensive efficacy and albuminuria improvement inpatients of primary hyperte nsion with renal function damage.
