摘 要:目的:探讨先天性心脏病右心室舒张性心力衰竭(DHF)患者心肌细胞内Ca2+超负荷、钙调蛋白及其基因表达的变化。 方法:采用逆转录聚合酶链反应(RT-PCR)和Westernblot技术测定10例先天性心脏病右心室DHF患者(DHF组)和6例正常对照(对照组)钙调蛋白及其基因的表达。 结果:DHF组患者心肌细胞内Ca2+含量较对照组高3倍以上,有非常显著性差异(P<0.01);肌浆网钙-三磷酸腺苷酶(SR Ca2+ATPase)和细胞膜L型Ca2+通道的信使核糖核酸(mRNA)水平较对照组减低,有显著性差异(P均<0.05),而肌浆网磷酸受纳蛋白、兰尼碱受体和肌集钙蛋白的mRNA表达较对照组无显著性差异;SR Ca2+ATPase蛋白的相对含量较对照组减低,有显著性差异(P<0.05=;磷酸受纳蛋白的相对含量无显著性差异。 结论:细胞膜L型Ca2+通道和SR Ca2+-ATPase基因表达减低是导致先天性心脏病心肌细胞内Ca2+超负荷和右心室DHF发生的主导因素。Objective:To elucidate molecular mechani sms underlying calcium handling in patients with right ventricular diastolic hea rt failure(RVDHF)from mRNA level and pro tein expression. Methods:Ten patients with RVDHF and 6 control subjects were enrolled in this s tudy. The mRNA amounts of these calcium handling genes were measured by reverse transcriptase polymerase chain reaction (RT-PCR)and normalized to the mRNA level s of actin.The protein levels of sarcopl asmic reticulum (SR)Ca2+- adenosine t riphosphatase(ATPase)and phospholamban w ere analyzed by western blot analysis. Results:The content of calcium was sig nificantly increased in patients with RV DHF(p<0.01=.RT-PCR analyses showed that the steady-state level of mRNA encoding the SR Ca2+-ATPase and cardiac L-type calcium channel were decreased significantly in patients with RVDHF(p<0.05).In contrast,no alterations of the mRNA levels for SR phospholamban,ryanodine receptor,calsequestrin were observed in these patients.Protein level of SR Ca2+-TPase was significantly reduced in patients with RVDHF,whereas protein level of phospholamban was unchanged. Conclusion:The expressions of L-type c alcium channel and SR Ca2+-ATPase wer e down regulated in patients with RVDHF. These changes may be a contributory fact or for RVDHF.
