血液透析滤过治疗尿毒症性周围神经病的疗效观察

摘　　要：目的　探求治疗尿毒症性周围神经病的有效方法。方法　将 40例患尿毒症性周围神经病并开始行维持性血液透析者随机分为观察组和对照组 ,观察组在每周 2次常规血液透析 (HD)的同时 ,接受每周 1次血液透析滤过 (HDF)治疗 ;对照组只行每周 3次常规HD治疗而不接受HDF治疗 ,治疗前、治疗 3个月时分别观察两组患者周围神经病的临床症状、神经反射改善情况及正中神经、胫神经的感觉神经传导速度 (SCV) 3项指标。结果　两组经过 3个月的充分透析后 ,其临床症状、神经反射均较治疗前有所改善 ,但两组间的差异无显著性 (P >0 0 5 ) ,而观察组的正中神经、胫神经SCV改善程度显著优于对照组 (P <0 0 5 )。结论　尿毒症性周围神经病患者在常规HD治疗的同时接受每周 1次的HDF治疗能更有效地改善其周围神经病变。Objective To investigate an effective treatment of uremic peripheral neuropathy.Methods Forty patients with uremic peripheral neuropathy were assigned randomly to study group and control group.The cases of study group received regular hemodialysis(HD) 2 times a week and hemodiafiltration(HDF) once a week.The control group received only regular HD 3 times a week.Before and after treatment for 3 months,the clinical symptoms,the nerve reflex and the sensory conduction velocity(SCV) of median nerve and tibial nerve were respectively observed in each group.Results After the hemodialysis for 3 months,the clinical symptoms and nerve reflex showed better than before,however,there was no significant difference in two groups(P>0 05).Whereas the SCV of median nerve and tibial nerve was enhanced faster in study group than that in control group,and there was significant difference(P<0 05).Conclusion The treatment of regular HD 2 times a week and HDF once a week can benefit the patients with uremic peripheral neuropathy.
