摘 要:目的 探讨经颅多谱勒(TCD)检测2型糖尿病脑血管病变的临床应用价值。方法 对71例糖尿病患者行TCD检查,并与对照组就频谱图、脉动指数(PI)、平均血流速度(Vm)参数的结果作比较分析。结果 TCD显示糖尿病组脑血管病变发生率为87.3%,明显高于对照组(P<0.001),且频谱图有特异性改变;糖尿病患者各检测动脉的PI值及颈内动脉颅外起始段(ICA)、大脑中动脉(MCA)、基底动脉(BA)的Vm值也显著高于对照组(P<0.001)。结论 TCD检测无症状2型糖尿病脑血管病变具有良好的敏感性。: Objective To evaluate the clinical value of transcranial Doppler ultrasonography (TCD) in detecting cerebral hemodynamic changes at the early stages of cerebrovascular disease in the diabetics. Methods TCD was used to measure the peak wave in the spectra, the pulsatility index (PI) and the mean blood flow velocity (Vm) of each cerebral artery in 71 normotensive type Ⅱ diabetic patients without clinical symptoms of cerebrovascular disease and 62 age- and gender-matched healthy controls. Results The incidence of cerebral hemodynamic changes detected with TCD was 87.3% in diabetic patients, whose PI for each cerebral artery was significantly higher than those in the controls. Significant increases in the Vm of the extracranial internal carotid artery, the middle cerebral artery and the basilar artery were detected in the diabetic patients when compared with the controls, suggesting lowered cerebrovascular compliance and smaller vascular diameter in the patients, which leads to indicate that the pathological changes had spread to the internal carotid artery and the vertebral artery system. Conclusion TCD can be helpful in detecting cerebral hemodynamic changes at the early stages of cerebrovascular disease in the diabetics.
