冠心病患者二尖瓣环运动频谱的研究

摘  要:目的 :用多普勒组织成像 (DTI)技术研究冠心病患者二尖瓣瓣环运动频谱。方法 :测量 2 0例正常人和 2 0例冠心病患者左室射血分数 (L VEF) ,二尖瓣口舒张期血流频谱 E波、A波和二尖瓣环频谱 Sa波、Ea波、Aa波的峰值速度。结果 :DTI脉冲多普勒测定的二尖瓣环频谱呈规律波群 ,有收缩期 Sa,舒张期 Ea和 Aa3个主波。冠心病组 Sa波、Ea波峰值速度及 Ea/Aa比值显著低于正常组。正常人 95 % Ea波起始早于血流 E波 ,而 70 %冠心病患者 Ea波晚于 E波。结论 :冠心病患者二尖瓣瓣环运动频谱与正常人的差异可反映其左室收缩和舒张功能之异常 ,提供瓣环机械动力障碍的信息。Objective: To study the motion of mitral annulus by using pulse doppler tissue imaging(DTI PWD). Methods: Left ventricular ejection fraction(LVEF), mitral inflow peak velocity(E,A), and mitral annular velocity(Sa, Ea, Aa) were measured with DTI PWD in 20 patients with coronary heart disease(CHD) and 20 normal subjects. Results: The spectrum pattern was similar in CHD group and normal group with three main waves in each cardiac cycle: Sa in systolic and Ea, Aa in diastolic. The peak velocities of Sa and Ea were significant lower as well as Ea/Aa ratio in CHD patients than that in normal group. The on sets of Ea were earlier than E in 95% normal subjects while in CHD group the on sets of Ea were later than E in 70% cases. Conclusion: Changes of mitral annulus motion spectrum derived from DTI PWD can reflect systolic and diastolic dysfunction of left ventricle, as well as they can provide information of mechanical abnormality.
