白毒伞中毒所致多器官损害33例分析

摘　　要：目的 :对 33例白毒伞中毒患者进行回顾性分析 ,探讨白毒伞中毒所致多器官损害的临床表现及治疗方法。方法 :对经卫生预防部门确定的 33例白毒伞中毒患者进行了肝、肾功能、三大常规检查 ,部分患者进行了心肌酶学、出凝血时间、DIC等检查 ,结合临床表现证实多脏器功能损害、衰竭表现为肝、肾、心、消化道、血液系统及神经系统等全身各器官组织损害 ,对肝脏损害尤为严重。其中 1例尸检还发现有内脏自溶。经对症综合治疗、血液净化、冷沉淀等治疗。结果 :33例中 2～ 4个脏器损害者 15例 ,治愈率 10 0 % ;5个脏器损害者 11例 ,治愈 3例 ,治愈率 2 7.3% ,死亡 6例 ,死亡率 5 4.5 % ,病情恶化自动出院 2例 (18.2 % ) ;6个脏器损害者 7例 ,死亡 6例 ,死亡率 85 .7% ,自动出院 1例。结论Objective:To retrospective analyze of amanita verna(AV) poisoning.Study amanita verna (AV) poisoing to result multiple organs damage,clinical situation and therapeutics method.Methods:to proceed liver function,renal function and three entry routine examinatiuon,part of patient proceed myocardic enzymoiogy,bleeding-clotting time,DIC et examination,To conjugate clinical situation confirmed multiple organs damage and functional failure.The mainly organs includes liver,kidney,heart,digestive tract,blood and neurological system and so on.Most of all,liver was the most seriously damaged organ.One of the cases was found entrails autolysis in autopsy.Results:These cases were treated with heteropathy,supporting treament,blood purification and crsults.All of 15 cases with two-four organs damage were healed (100%).3 of 11 cases with five organs damage ere healed(27.3%),and 6 died (54.5%),2 cases of auto-out-hospital because of pejorative(18.2%),6 of 8 cases with six organs damage died (85.7%),and 1 case of auto-hospital.Conclusions:To accompany the more organs damage numbe to increase the progressivelier death-rate.
