β_2-微球蛋白在强直性脊柱炎肾损害早期诊断中的临床评价

摘　　要：目的 :探讨 β2 微球蛋白 (β2 -MG)在强直性脊柱炎 (AS)肾损害早期诊断中的价值。方法 :78例AS患者按传统肾损害标准 (尿液镜检异常 ,血尿素氮 >7.1mmol/L ,血肌酐 >133.3μmol/L)分成无肾损害组及有肾损害组。另取 30例健康人为对照组。采用放射免疫法 (RIA)分别对各组进行血、尿β2 -MG含量检测和对比分析。结果 :AS有肾损害组与无肾损害组血、尿 β2 -MG含量明显升高于对照组 (分别为P <0 .0 0 5及P <0 .0 5 ) ,AS有肾损害组较无肾损害组血、尿 β2 -MG含量显著升高 (P <0 .0 5 )而AS无肾损害组血、尿 β2 -MG含量较对照组亦明显升高 (P <0 .0 5 )。结论 :AS患者存在着肾脏损害 ,监测AS患者血、尿 β2 -MG可作为AS肾损害早期诊断的敏感指标 ,对AS的治疗和预防AS肾功能损害的发生。Objective:Probe into the early diagnostic value of β 2-Microglobulin (β 2-MG)in ankylosing sponeylitis (AS) kidney damage.Methods:78 cases of AS were devided into non-kidney damage group and kidney damage group by the traditional kidney damage creterion (urine is abnormal by microscope test,blood urea nitrogen(BUN),excesses 7.1mmol/L and serum creatinine (CR) 133.3umol/L) and another 30 healthy beings were selected as the contrasting group.The three groups were respectively tested the β 2-MG content in blood and urine by radioimmunoassay (RIA),and the analyzing figures emerged.Results:By analyzation the β 2-MG content kidney damage group was much higher than in the contrasting group (P<0.005,P<0.05).The β 2-MG content in AS kidney damage group was also much higher than in the non-kidney damage group,while the β 2-MG content in blood and urine of AS non-kidney damage was much higher than in the contrasting group (P<0.05).Conclusions:There is kidney-damage in AS patients.β 2-MG patients blood and urine can be considered as the sensitive index in the early diagnose of As kidney damage,meanwhile it will be of great significance in the treatment,improvement of prognosis and prevention of AS from happening and developing.
