摘  要:目的 :对左心辅助的两种插管引流途径 (左心室引流与左心房引流 )进行对比研究 ,为临床选择合适的引流途径提供实验依据。方法 :12条犬分别建立左心房 -主动脉 (L A组 )和左心室 -主动脉 (L V组 )的左室辅助动物模型 ,并进行血流动力学和超声心动图检查。结果 :L V组的左室收缩压、平均压及压力变化速率均显著低于L A组。辅助后两组的左室、左房与右室内径均减小 ,但 L V组左室内径减小更明显。两组的二尖瓣 (E峰 )及主动脉瓣的流速无显著差异。结论 :两种引流途径均有较好的血流动力学效应。左心室引流时流量大 ,左室减压更充分 ;左心房引流对心脏创伤小 ,出血并发症少。临床应用时 。Objective:To compare the effects of two types of cannulation sites on the hemodynamics of left ventriclar assistance to provide experimental basis for proper selection of cannulation sites clinically.Methods:Twelve mongrel dogs were randomized equally into the left atrium to aorta group(the LA group)and left ventricle to aorta group (the LV group)and underwent hemodynamic and echocardiographic evaluations.Results:Of the LV group,the left ventricular systolic pressure,mean left ventricular pressure and ±dp/dt were significantly lower than those of the LA group.The dimensions of the left ventricle,left atrium and right ventricle diminished in both groups,while the left ventricular internal diameter of the LV group reduced more remarkably.There were no significant distinctions between the velocities of the aotic valve and mitral valve(E peak) in both groups.Conclusions:Both cannulation methods have satisfactory hemodynamic effects while left ventricular cannulation has a greater flow rate and a more sufficient decompression of the left ventricle resultantly. Left atrium cannulation is less traumatic and causes less hemorrhage.Different cannulation sites are preferred in different clinical settings.
