摘 要:目的 :探讨高血压患者冠状循环的改变。方法 :采用经食管超声心动图 (TEE)技术 ,探测 30例高血压患者和 10例对照的冠状窦解剖结构和血流动力学变化。结果 :与对照组相比 ,高血压病患者收缩压 (SBP)、舒张压 (DBP)、左室重量指数 (L VMI)、冠状窦前向血流 (CSF)及冠状窦直径明显增加 (P<0 .0 5~ 0 .0 0 1) ;与对照组相比 ,心肌肥厚组 L VMI、SBP、 DBP、冠状窦直径、 CSF均明显增加 (P<0 .0 5~ 0 .0 0 1) ;而无心肌肥厚组仅 L VMI明显增加 (P<0 .0 1) ;与无心肌肥厚组相比 ,心肌肥厚组 L VMI、 SBP、 CSF明显增加 (P<0 .0 5~ 0 .0 0 1)。结论 :高血压患者冠状循环发生了显著改变 ,左室心肌重量增加是引起这一改变的主要原因 ,应用Objective:To evaluate characteristic changes of coronary circulation in patients with hypertension.Methods:Thirty patients with hypertension and ten control subjects were studied by transesophageal coronary sinus Doppler echocardiography.Peak systolic and diastolic and retrograde coronary sinus flow velocity were measured,including respective flow velocity time integrals(VTI S,VTI D,VTI A).VTInet was calculated by subtracting the retrograde from the anterograde flow velocity time integral(VTI S+VTI D-CTI A),and VTInet×Heart rate was calculated as an index of coronary sinus flow (CSF).Results:Patients with hypertension had higher systolic and diastolic blood pressure,left ventricular mass index,CSF and coronary sinus diameter,compared to those of control subjects.Hypertensive patients with left ventricular hypertrophy(LVH)had higher left ventricular index,systolic blood pressure and CSF,compared to those of hypertensive patients without LVH.Conclusions:Patients with hypertension have abnormally high coronary sinus flow due to increasing left ventricular mass.It is suggested that estimation of coronary circulation by transesophageal coronary sinus Doppler measurements is feasible and has significant value in diagnosis of hypertension.
