原发性高血压不同病程对左室形态及心功能影响的研究

摘　　要：目的　探讨原发性高血压不同病程对左室形态及功能的影响。方法　检测了 113例 (病程 <5年者 43例、≥5年者 70例 )高血压患者的左室结构及功能。结果　与对照组和病程 <5年者相比 ,病程≥ 5年者其左房指数 (L AI)、室间隔厚度 (IVSd)、左室后壁厚度 (PWTd)、左室舒张末期内径 (L VIDd)、左室质量 (L VM)均明显增大 ;与正常对照组相比 ,两组高血压患者的收缩和舒张功能均减低 ;两组患者之间比较除 L VEF有显著差异外 ,其于指标无统计学意义。结论　高血压病早期即可致心脏结构和功能的改变 ,但病程越长 ,似乎其收缩功能的损害更为突出。Objective To evaluate the influence of primary hypertension with varied course of disease on left ventricular configuration and function.Methods We measured the left ventricular configuration and function by means of echocardiography and radionuclide test in 113 hypertension patients (duration<5 years,43 cases;≥5years,70cases).Results The LAI,IVSd,PWTd,LVIDd and LVM of hypertension group(duration≥5 years)were significantly increased as compared with those of control group and patient(duration<5 years)group.The systolic and diastolic functions were both impaired in the two hypertension groups,but the comparison between these two groups showed that significant difference was noted only in LVEF.Conclusion Hypertension can lead to change in heart configuration and function at early stage;the longer the course of disease lasts,the more severe will be the impairment of systolic function,and the more prominent will be the change in left ventricular configuration.
