三种内固定在开放性胫腓骨骨折中应用

摘　　要：目的　观察开放性胫腓骨骨折三种内固定的疗效。方法　开放性胫腓骨骨折 ,彻底清创后分别予普通钢板螺钉、矩形钉、交锁髓内钉内固定。结果　随访 5～ 72个月 ,术后深部感染 :普通钢板螺钉组 8. 2 %、双矩形钉组 5 . 3%、交锁髓内钉组 3. 2 % ;内固定失效率 :普通钢板螺钉组 19. 1%、双矩形钉组 14. 5 %、交锁髓内钉组 3. 2 % ;术后患肢关节活动优良率 :普通钢板螺钉组 86 . 4%、双矩形钉组 93. 4%、交锁髓内钉组 10 0 %。结论　交锁髓内钉治疗开放性胫腓骨骨折远期疗效满意 ,双矩形钉次之 ,普通钢板螺钉差。Objective To study the value of three internal fixation devices (common plate,rectangle shape intramedullary nail and interlocking intramedullary nail)for treating open fracture of tibia. Method All the patients were divided into three groups randomly.One handred and ten patients were treated with common plate,76 patients were treated with rectangle shape intramedullary nail,31 patients were terated Interlocking intramedullary nail.After the three internal fixation devices were used in the open fracture of tibia.Results All the patients were followed up from 5 months to 72 months.The rate of postoperative depth inflammation were 8.2%,5.3% and 3.2% respectively.The rate of the internal fixation failure were 19.1%,14.5% and3.2% respectively,the rate of excellent and good functin of knee and ankle joints were 86.4%,93.4% and 100% in the samples.Conclusion The result showed that the long term effect of interlocking intramedullary nail was more satisfactory than that of common plate and rectangle shape intramedullary nail for treating open fracture of tibia.
