摘  要:目的 了解椎间盘摘除后与BAK置入后椎间关节内的应力变化 ,为临床开展BAK置入术防止关节突增生提供实验依据。方法 使用自身对照法 ,用 6具健康成年人腰1~腰5椎体标本 ,12个直径 14mm、长 2 5mm的BAK ,在腰4~ 5椎间盘摘除术前后及置入BAK后 ,用微型传感器测试椎间关节内应力变化。结果 椎间盘摘除后 ,2 0 0 0N轴向载荷下椎间关节内应力较椎间盘摘除前显著上升 ;而BAK置入后 ,同样载荷下椎间关节内应力则与椎间盘摘除前无显著差别。后伸力矩作用下 ,椎间盘摘除后椎间关节内的应力比术前显著上升 ;而BAK置入后 ,则显著减小。侧弯力矩作用下 ,椎间盘摘除后 ,椎间关节内应力却下降 ,并随着力矩的增加 ,椎间关节内应力变化率逐渐减小。而BAK置入后 ,应力则明显减小。结论 椎间盘摘除术后 ,椎间关节内应力显著增加 ,可能是关节突增生的主要原因 ;而BAK的置入可明显减小椎间关节内的压应力 ,因而可预防关节的增生。Objective In order to understand the stress changes of intervertebral joint facet after discectomy and BAK implant operation,to probide experiment data for clinic developing BAK implant operation to prevent the articular process proliferation.Methods Using self-correlative study,it was used that the six healthy adult specimens and twelve modeling BAKs diameter 14 mm and length 25 mm.After the L 4,5 discectomy and the BAK implant operation,the stress changes of intervertebral joint facet was tested by miniature transducer.Results Under 2000N axial load,the stress of intervertebral joint facet rose higher before discectomy than after discectomy.After BAK implant the press returned the pre-discectomy level.Under posterior extension momentum the press of intervertebral joint facet rose largely after discectomy,but the press stress returned the pre-discectomy level after BAK implant.Under lateral bending momentum the stress of intervertebral join facet decreased after discectomy and the press change rate became small.After BAK implant,the press had decreaced.Conclusion The press stress of intervertebral joint facet rose notably after discectomy and decreased obviously after BAK implant,so,it can prevent the proliferation of articular process.
