急性心肌梗死病人血浆补体活化成分与IL-2R水平的变化

摘　　要：①目的　探讨急性心肌梗死 (AMI)病人血浆补体活化成分 (sC5b 9)及白细胞介素 2 (IL 2 )水平的变化及意义。②方法　采用酶联免疫吸附法 (ELISA)和碱性磷酸酶抗碱性磷酸酶法 (APAAP) ,检测 6 7例AMI病人发病第 1,4,7天时和 38例健康人的sC5b 9,可溶性白细胞介素 2受体 (sIL 2R)、淋巴细胞膜白细胞介素 2受体(mIL 2R)表达的变化。③结果　AMI病人sIL 2R ,sC5b 9明显增高 ,mIL 2R表达明显降低 ,与对照组比较差异有极显著意义 (F =18.6 73～ 36 .44 5 ,q=6 .6 82～ 16 .416 ,P <0 .0 0 1)。死亡者和伴有室性心律失常 (VA)者sIL 2R ,mIL 2R ,sC5b 9变化较存活者和不伴有VA者更明显 (t=7.6 93～ 12 .393,P <0 .0 0 1)。AMI病人sC5b 9与sIL 2R呈正相关 (r =0 .6 17,P <0 .0 0 1) ,与mIL 2R呈负相关 (r =- 0 .6 80 ,P <0 .0 0 1)。④结论　AMI病人sIL 2R ,sC5b 9水平和mIL 2R表达明显异常 。Objective To explore the changes of soluble composition of complement activation (sC5b 9) and interleukin 2 receptor in patients with acute myocardial infarction(AMI) and its clinical significance. Methods By means of enzyme linked immunosorbent assay(ELISA) and alkaline phosphatase antialkaline phosphatase(APAAP),the changes of sC5b 9, soluble interleukin 2 receptor(sIL 2R) and lymphocyte membrane interleukin 2 receptor(mIL 2R) were measured on the 1st , 4th and 7th day after the onset of AMI. Results The sC5b 9 and sIL 2R were higher, mIL 2R was lower for the patients than those for the controls(F=18.673-36.445,q=6.682-16.416,P<0.001).The parameters changed more markedly with the advance of the disease. The changes of sIL 2R and sC5b 9 in patients with ventricular arrhythemia(VA) and the deaths were more evident than those of survivors and those without VA(t=7.693-11.306,P<0.001). The sC5b 9 was positively correlated with sIL 2R and negatively correlated with mIL 2R in AMI patients (r=0.617,-0.680,P<0.001). Conclusion The sC5b 9, sIL 2R and mIL 2R were markedly abnormal in AMI patients and closely related to the seriousness and prognosis.
