慢性前列腺炎难治原因初步分析

摘　　要：目的 :初步分析难治性慢性前列腺炎难以治愈的原因。　方法 :3 80例慢性前列腺炎病人予以综合治疗 ,分析易治与难治组一般相关资料 ,并行前列腺液病原体检测、药物敏感试验、β 内酰胺酶定性测定 ,及性功能、焦虑、抑郁表格评分。　结果 :3 80例中 12 8例治愈 ,2 5 2例未治愈。难治组的包皮过长或包茎、不洁性生活接触史、治疗过程中再次不洁性生活接触史、前列腺结石、性伴侣未治疗、抗生素滥用 ,病原体检出率、混合病原体感染率 ,金黄色葡萄球菌、大肠埃希菌、淋病奈瑟菌及表皮葡萄球菌感染率 ,产生 β 内酰胺酶的金黄色葡萄球菌及表皮葡萄球菌和焦虑、抑郁评分明显高于易治组 ,而性功能评分明显低于易治组。　结论 :抗生素滥用、耐药菌株增殖、外来菌侵袭、多种病原体混合感染、病原体再次感染及前列腺液瘀积 ,可能是难治性慢性前列腺炎难以治愈的原因。Objectives: To study the incurable causes of chronic prostatitis preliminarily. Methods: 380 cases of chronic prostatitis were treated and then were divided into curable group and incurable group. General related materials were analyzed, and detection of pathogens in prostatic secretions, sensitive tests of drug, qualitative analysis of β-Lactamases and scores of sexual function, anxiety and depression were performed. Results: 128 cases were cured and 252 cases were not. The cases of phimosis or redundant prepuce, contact history of extramarital intercourse and contact history of extramarital intercourse again in the course of treatment, stone of prostate, no treatment to sexual partner, abuse of antibiotics, detective rate of pathogens, infective rate of many pathogens, Staphylococcus aureus, Escherichia coli, Neisseria gonorrhoeae and Staphylococcus epidermidis, number of Staphylococcus aureus and Staphylococcus epidermidis to produce β-Lactamase and scores of anxiety and depression were more in incurable group than that in curable group, but the score of sexual function was less in incurable group than that in curable group. Conclusions: The abuse of antibiotics, reproduction of drug-fast bacteria, invasion of external bacteria, mixed infection of many pathogens, second infection of pathogens and deposition of prostatic secretion were the causes of incurable chronic prostatitis.
