摘 要:目的 探讨膝关节色素沉着绒毛结节滑膜炎 (PVNS)的发病与骨破坏机制、影像特征。方法 回顾性研究 8例PNVS形态、信号、骨侵蚀表现及强化特点。结果 结节T1WI等信号 6例 ,T2WI等信号 3例 ,3例见灶性低信号。小囊状破坏 5例 ,1例股骨关节面破坏。 4例MR增强见相对较弱强化区。结论 PVNS与巨噬细胞化生为滑膜细胞和成纤维细胞有关。MRI信号取决于含铁血黄素的比例。骨侵蚀可能与多核巨细胞的类破骨细胞特性有关。Purpose To research the pathogenesis and mechanism of bone erosion and the imaging characters of pigmented villonodular synovitis(PVNS) of the knee. Methods 8 cases of MRI and CT finding were reviewed. MRI Signal intensity features, morphological characters and bone erosion were observed. Results On T1WI, the signal of villous nodule of 6 cases was iso-intensity. On T2 WI, the signal intensity of 3 cases was iso-intensity, the remainder 3 cases was low intensity in lesions. Five cases were cystic-like destruction, another case involved in the condyle of femur. On contrast enhanced MR imaging, focal relative weakly enhanced region were found in 4 cases. Conclusion The cause of PVNS was related to the shift of macrophages to B synoviocytes and fibroblasts. MR finding depended on the proportion of hemosiderin. The bone erosion probably caused by multinuncleated cell, which expressed phenotypic feature of osteoclasts.
