摘 要:目的 观察增生性肾炎患者肾小球系膜细胞 (MC)在不同增生程度时细胞周期调控蛋白表达水平的变化。方法 采用免疫组织化学技术 ,检测 34例肾小球肾炎患者肾穿刺标本中细胞周期正性调控蛋白周期素D1(cyclinD1)、负性调控蛋白P2 1CIP1(P2 1)、P2 7KIP1(P2 7)和增殖细胞核抗原(PCNA)表达。结果 在正常对照及非增生性肾炎组中 ,cyclinD1、P2 1不表达 ,P2 7呈高表达。在增生性肾炎组中 ,cyclinD1、P2 1和P2 7阳性表达主要在系膜区 ;随着系膜增生程度的加重 ,肾小球cyclinD1、P2 1表达增加 ,并与PCNA呈正相关 (P均 <0 0 1) ;P2 7则表达减少并与PCNA呈负相关 (P <0 0 5 ) ;肾小球P2 7表达还与血肌酐之间呈显著性负相关 (P <0 0 1)。结论 在人类增生性肾炎中 ,cyclinD1、P2 1和P2 7参与MC的增生 ;P2Objective To observe the expression of the cell cycle regulatory proteins in mesangial cells (MC) of proliferative nephritis, and to determine the molecular pathological significance of the cell cycle regulatory proteins in the mesangial cell proliferation in the proliferative nephritis. Methods Immunohistochemical studies were performed on 34 specimens from renal biopsies to detect cyclin D1, P21 CIP1 (P21), P27 KIP1 (P27) and proliferating nuclear antigen (PCNA). The correlations among cyclin D1, P21, P27 and PCNA, the degree of MC proliferation and clinical index were analyzed. Results In normal control and non proliferative nephritis groups, stainings for PCNA were very rare, and stainings for cyclin D1 and P21 were absent. While P27 was expressed abundantly. In proliferative nephritis groups, cyclin D1, P21 and P27 were mainly expressed in mesangial cells. The more severity of the mesangial proliferation, the more increasing of the expression of PCNA, cyclin D1 and P21, and the more decreasing of the expression of P27. Total staining positive cells of cyclin D1 and P21 showed a highly significant correlation with that of PCNA (all P <0.01). A negative correlation between P27 with PCNA and the serum creatinine (all P <0.05) was revealed. Conclusions In human proliferative nephritis, cyclin D1, P21 and P27 may play an important role in regulating the proliferation of mesangial cells. P27 may highly relate to the progress of renal diseases.
