肾脏疾病患儿sIL-2R测定及其临床意义探讨

摘  要:目的 探讨肾脏疾病患儿可溶性白细胞介素 2受体 (sIL 2R)和T细胞亚群的改变及其临床意义。方法 用ELISA法检测 62例肾病综合征、48例肾小球肾炎患儿及 3 0例健康对照儿童血清和尿中sIL 2R浓度 ,单克隆抗体间接免疫荧光法检测T细胞亚群。结果  (1)肾病综合征及肾小球肾炎活动期组血清及尿sIL 2R浓度分别高于各自缓解期组 (P <0 .0 5 )和对照组 (P <0 .0 5 ) ;各自缓解期组血清和尿sIL 2R浓度与对照组间差异无显著意义 (P >0 .0 5 )。 (2 )肾病综合征和肾小球肾炎活动期患儿尿与血清中sIL 2R浓度呈正相关 (P <0 .0 1)。 (3 )肾病综合征活动期组CD4 + 及CD4 + /CD8+ 明显低于该病缓解期组和对照组 (均P <0 .0 1)。结论 肾病综合征和肾小球肾炎活动期患儿T细胞免疫应答能力低下 ,sILObjective To detect the changes of soluble interleukin 2 receptor(sIL 2R)level and T cells in children with nephroses.Methods The levels of sIL 2R and T cells in serum and urine in 62 children with nephrotic syndrome,48 with glomerular nephritis and 30 controls were measured by ELISA and monoclonal antibody indirect immunofluorescence test respectively.Results The levels of sIL 2R in serum and urine were significant higher in active stage of nephrotic syndrome and glomerular nephritis than in their remission stage( P <0.05) and controls( P < 0.01 ).There was no significant differences between the remission stage and the controls( P >0.05).The level of sIL 2R in urine was positively correlated to that in serum in nephritic active stage of syndrome and glomerular nephritis.The percent of CD 4 + cells and CD 4 +/CD 8 + were significantly lower in active stage of nephrotic syndrome than in its remission stage and the controls( P <0.01).Conclusion It is suggested that the ability of immune response decrease in active stage of nephritic syndrome and glomerular nephritis and the elevation of sIL 2R level may be used as an index of nephroses activation.
