北京市汽南社区高血压患者强化管理的近期效果评估

摘　　要：目的　对汽南社区高血压患者 2年强化管理的效果进行评估。方法　对所管居委会按随机整群分层抽样的方法 ,分成干预组和对照组。干预组采用健康教育→高血压知晓率提高→卫生知识知晓水平提高→行为改变→管理率、控制率、规范服药率提高→健康状况改善的强化管理方法 ;对照组采用门诊随访的管理方法 ;通过前瞻性研究观察干预组和对照组自身前后及在各时间点组间比。结果　干顾组通过社区卫生服务站全科医生的管理 ,高血压知晓率由 36 89%达到 98 93 % ,管理率由 30 6 4%提高至 98 93% ,控制率由 16 16 %提高至85 5 2 % ,规范服药率由 31 4%提高至 97 2 % ,SBP净降 2 7 77mmHg ,DBP净降 7 2 9mmHg ,与对照组及自身干预前比较差异有非常显著性的意义 (P <0 0 1)。结论　利用社区卫生服务站进行社区人群以高血压知晓率提高为突破点的综合防治更能有效提高管理率、控制率 ,使高血压患者健康状况改善 。Objective Effect of the two-years intensified administration of hypertension suffers in Qinan community was evaluated.Methods With the method of delimitation sample among the whole group,the residents in Qinan residential committee was divided into interferential group and compared group.In the interferential group,methods of intensified administration,such as health education→increase of the ratios of knowing hypertension→increase of knowledge→behavior change→increase of ratio of administration,control ratio, ratio of criterion doses→improvement of health status;In the compared group follow-up visit measures in out-patient clinic were adopted;through preceding research,we had observed the interferential group and compared group and ratios of each group each time.Results Through the administration by all around doctors in the Sanitation Service Center in the community,ratio of knowledge of hypertension was raised from 36 89% to 98 93%→ratio of administration from 30 64% to 98 93%,control ratio from 6 16% to 85 52% ratio of criterion doses from 31 4% to 97 2%→improvement of health status;SBP was decreased to 27 77mmHg entirely,DBP 7 29mmHg entirely,in interferential group.Compared with the compared group and the time before interference,the differences were notably prominence(P<0 01).Conclusion With sanitation service center in the community,we have taken all-round prevention and cure onto people in the community,as the increase of ratio of knowledge of hypertension,so that we could do our best to increase the ratio of administration,control ratio,improving health status of the hypertension suffers as well as reducing cases of cardiopathy and brain blood vessel.
