庆大霉素应用在气腹时的肾毒性实验研究

摘  要:目的 :研究气腹是否会加强大鼠的庆大霉素的肾毒性。方法 :收集 2 8只大鼠中 2 4 h的尿液。气腹组 ( n=7) ,在 15 m m Hg压力下持续 2 h腹腔注入二氧化碳。庆大霉素组 ( n=7) ,静脉注射 9m g/kg庆大霉素。气腹结合庆大霉素组 ( n=7) ,在气腹前10 m in给相同剂量的庆大霉素。操作后 ,收集 2 4 h尿液 ,7d后检测血液肌酐并收集最后 2 4 h尿液。对所有的尿液样品进行肌酐和 NAG( N- acetyl- beta- glucosam inidase)的定量分析。结果 :庆大霉素组和气腹结合庆大霉素这两组表现出第一天的 NAG排泄明显大于同一天空白组或操作前的量。第一天和第七天尿液的剩余肌氨酸酐和 NAG的量同一天空白组和操作前对应的量没有显著的差别。在这些组中第七天的血清肌酐和肌酐清除率没有显著差异。结论 :给大鼠静脉注射庆大霉素会使尿液的NAG排出量暂时性增加 ,NAG排出量在 7d内达到稳定。单独 2 h 15 m m Hg气腹或合并庆大霉素治疗不会增加大鼠的尿液NAG。而且肯定了庆大霉素加气腹不会使肌酐清除量减少超过 60 %。这些结果不支持“气腹会加强庆大霉素的肾毒性”的假设。OBJECTIVE:To study whether pneumascos combined with gentamicin aggravates mice's nephrotoxicity. METHOD: Twenty eight mice's urine in 24 hours was collected and then divided the mice into 4 average groups. The first group were just anaesthetized while the second infused CO 2 into their abdomens under the pressure of 15mmHg. The third group were intravenated gentamicin (9mg/kg) and fourth intravanated the same amount of gentamicin 10 minutes before being infused CO 2. The urine within the first 24 hours was collected. Then blood creatinine was assayed 7 days later and urine within the last 24 hours was also collected. At last, creatinine and NAG in all urine were analysed.RESULT:The latter two groups' urine on the first day had much more NAG than that the first one, but their amount of creatinine and NAG on the first day was almost the same as that on the 7th day. While in the 7th day's urine, there was no various difference in blood creatinine and Ccr between these groups.CONCLUSION: There was a temporary increase in NAG in mice's urine after they were intravenated gentamicin and NAG reached its top amount in 7 days. The combination of pneumascos and gentamicin did not increase NAG in urine.
