摘  要:目的 :研究再生障碍性贫血 (AA)发病机制及环孢菌素 A(Cs A)对 AA患者骨髓细胞体外培养的影响。方法 :采用微量甲基纤维素法研究了 18例 AA患者骨髓粒—单系祖细胞 (CFU- GM)集落形成及其骨髓细胞和血清对正常 CFU- GM集落形成的影响 ,据此对 AA按发病机制进行分组 ,同时加 Cs A与 AA骨髓及正常骨髓共同培养。结果 :干细胞缺陷组 5例 ,微环境缺陷组 4例 ,细胞抑制组 7例 ,血清抑制组 2例。 AA骨髓 CFU - GM集落产率明显低于正常对照 (P<0 .0 1) ;Cs A能显著增加 AA集落、集族产率 (P<0 .0 5 ) ;8例 AA骨髓对 Cs A敏感 ,其中细胞抑制组 6例 ,干细胞缺陷组 2例。结论 :本研究可能为预测 Cs A治疗 AA的临床疗效提供实验依据。Objective:To explore the pathogenesis of aplastic anemia (AA) and the influence of cyclosporin A (CsA) on bone marrow (BM) cells from AA patients in vitro.Methods:Eighteen AA patients were tested to observe the BM CFU GM colony formation and the influence of AA BM cells and sera on normal BM CFU GM growth by using methylcellulose micromethod.According to this the pathogenesis of AA was classified.At the same time CsA was cocultured with AA and normal BM.Results:There were 5 cases with stem cells defect,7 cases with cells suppression,2 cases with sera suppression and 4 cases with microenvironmental defect.The BM CFU GM colony formation of AA was significantly lower than that of normal control ( P <0.01).CsA could increase the colony and cluster formation of AA group ( P <0.05).8 cases of AA BM were sensitive to CsA.Among them there were 6 cases with cell suppression and 2 cases with stem cells defect.Conclusion:This study would provide experimental basis for predicting clinical curative effect of CsA on AA.
