丹仙康骨胶囊对激素性股骨头缺血坏死血运变化的影响

摘　　要：目的 :探讨经验方制剂丹仙康骨胶囊治疗激素性股骨头缺血坏死的作用机理 ,为临床用药提供理论依据。方法 :6月龄中国家兔 4 6只 ,分为正常组、模型组、中药治疗组。模型组与治疗组用氢化醋酸泼尼松肌注造模 ,A组肌注生理盐水。实验四周后正常组、模型组以生理盐水、中药治疗组以丹仙康胶囊灌胃 ,分别于治疗后 4、8、1 2周分批处死。测定血清总胆固醇 ,甘油三酯与高密度脂蛋白 ,观察股骨头血运变化 ,并以肝脏 ,睾丸 ,卵巢进行组织形态学观察。结果 :中药治疗组与模型组比较 ,其股骨头墨染增多 ,骨陷窝空虚数减少 ,骨髓纤维增生 ,坏死骨小梁周围可见到增生的成骨细胞 ,血清总胆固醇 ,甘油三酯均显著降低 ,而高密度脂蛋白则增高 ( P<0 .0 5或 0 .0 1 ) ,肝细胞脂肪样变减轻 ,可见到肝索结构 ,睾丸卵巢纤维化好转。结论 :丹仙康骨胶囊能改善病变股骨头内微循环与促进成骨细胞的分化增殖 ,从而加速骨坏死修复过程。Objective:Study on the effets of Dan Xian Gu Jiao Nang(DXKGJN) in treatment of steroid induced avascular necrosis of femoral head in order to provide theory for clinical use.Methods:46 chinese rabbits aged 6 months were divided randomly into three groups:normal group(15),model group(15) and DXKGJN group(16).The normal group were administered intramusculary with normal saline. The rest two groups were administered intramuscularly with steroid.After 4 weeks, DXKGJN group were treated with DXKGJN, and other two groups with normal saline. Every group were killed in batches at 4th,8th,12th week from treatment time.Serologica tests(cholesterol,triglyceride and high density lipoprotein) and histological studies of femoral head, hepatis and testis or ovary were performed.Results:The serum cholesterol and TG were lower significantly in the DXKGJN group than those in the model group,but the high density lipoprotein was contrary.Compared with model group, DXKGJN group significantly showed followings:Area dyed by ink increased;Numbers of empty bone lacuna decreased;Fibroblasts multiplicated in the margin of marrow;Osteoblasts multiplicated aroud the necrosis of trabecula; Hepatic cell fatty degeneration mitigated so clearly that hepatic cell cord could be seen;The atrophy of testis and ovary mitigated.Conclusion:Through ameliorating the microcirculation in femoral head and promoting osteoblast multiplication,DXKGJN could speed up the repair process of avascular necrosis of femoral head.
