摘 要:目的 :探讨三维动态增强磁共振血管成像 (3DDCEMRA)技术及其对主动脉夹层的应用价值。方法 :前瞻性研究 73例疑主动脉夹层患者 ,均行 3DDCEMRA检查 (重复时间 9ms,回波时间 4ms或 5ms,翻转角 60°) ,对比剂用量 0 .1 5~ 0 .2mmol/kg,先行减影处理 ,再行最大信号强度投影 (MIP)重建。以手术和 /或综合检查为标准 ,评价 3DDCEMRA诊断结果。结果 :手术和 /或综合检查发现AD 60例 ,3DDCEMRA检出 58例 ,其敏感性 97% ,特异性 1 0 0 %。 58例均显示了内膜片及真假腔并正确分型 ,显示内膜破口 54个 ,对主动脉分支血管受累评价准确率 89% ,血栓形成表现为假腔内造影剂充盈缺损。结论 :3DDCEMRA扫描时间短 。Objective:To study the method of 3D DCE MRA and its clinical application for aortic dissection(AD).Methods:Seventy three patients with AD underwent 3D DCE MRA of aorta with administration of Gd DTPA at 0.15(0.2 mmol/kg body weight using the following acquisition parameters: a TR of 9 ms, TE of 4 ms or 5ms,flip angle of 60°. The source images were subtracted from mask images at first,and followed by maximum intensity projection (MIP) reconstruction. Following the criteria established on the surgical findings or the results of the comprehensive imaging exams, the results of 3D DCE MRA were evaluated.Results:There were 60 cases with AD proved by surgical findings and/or the comprehensive imaging exams,and of which 58 cases were detected by 3D DCE MRA. The sensitivity and specificity of 3D DCE MRA were 97% and 100%,respectively. In 58 positive cases,intimal flap, the true and false lumen and 54 entry tears were shown. Involvement of aortic branches was shown in 65 cases(89% accuracy ). Mural thrombi were demonstrated by means of filling defect of the false lumen. Conclusion: 3D DCE MRA can satisfy the majority of diagnostic needs of aortic dissections with short scanning time.
