彩色多普勒超声对原位肝移植患者血流动力学的研究

摘　　要：目的 :应用彩色多普勒血流显像 (CDFI)评价原位肝移植 (OL T)患者的肝血流动力学的改变。方法 :85例 OL T患者 ,于术前、术中、术后应用 CDFI测定肝总血流量 (TL BF)、血流灌注指数 (DPI)、门静脉充血指数 (PCI)、阻力比 (RR)等血流参数 ,并与 30例正常人对照。结果 :1OL T患者术后七天之内肝动脉峰值流速明显低于正常对照组 (P<0 .0 1) ;2术后七天之内门静脉平均流速略高于正常对照组 (P<0 .0 5 ) ;3其它各项血流动力学参数与正常对照组无显著性差异 (P>0 .0 5 ) ;4CDFI对 OL T术后肝动脉 (HA)狭窄、门静脉 (PV)、腔静脉 (IVC)血栓及胆道并发症可做出较为特异性的诊断。结论 :CDFI对 OL T术前患者的筛选。Objective:To evaluate the changes of hepatic hemodynamics of 85 cases with orthotopic liver transplantation(OLT)by colour Doppler flow imaging(CDFI).Methods:To measure the total liver blood flow(TLBF),Doppler perfusion index(DPI),Portal congestive index(PCI),and resistance ratio index(RR)of the OLT and 30 healthy persons as the controls by CDFI.Results:1)The peak velocity in the hepatic artery(HA)of the patients was obviously lower than that of the controls within 7 days after the operation(P<0.01);2)The mean velocity of portal vein(PV)was higher than that of the controls within 7 days ofter the operation(P<0.05).3)As to all the other parameters of hemodynamics,there were no significant differences between the patients and the controls.4)CDFI could help to diagnose the complications of OLT specifically,especially HA strictness,Portal vein and inferior vena cava thrombi and biliary duct complications.Conclusions:CDFI can provide main bases for sifting liver transplantation patients before operation,monitor hemodynamics and diagnose complications in the early stage after operation.
