摘  要:目的 研究老年轻度认知功能损害者的脑形态结构变化特点。方法 于 1999年 1月至 2 0 0 0年 6月用脑磁共振显像 (MRI)三维测量法组织分割和体积测量分析技术 ,检查有轻度认知功能损害的老年人 2 1例 (MCI组 )和认知功能正常的老年人 2 9名 (NC组 )。结果  (1)MRI测量 :与NC组相比 ,MCI组的颅内总体积少 5 2 % ,灰质体积少 8 8% ,灰质百分比少 3 9% ,总脑脊液多 12 3% ,两组差异有显著或非常显著性 (P <0 0 5或P <0 0 1)。 (2 )多元逐步判别分析 :脑灰质和侧脑室体积具有非常显著性判别意义 (P <0 0 1) ,两组判别总正确率为 74 5 %。Objective To Assess the brain structural changes in the elderly with mild cognitive impairment using 3 dimentional magnetic resonance images Methods The elderly subjects with mild cognitive impairment (MCI) and normal controls (NC) were measured with a computerized volumetry and tissue segmentation program to quantify volumes of hippocampal formation (HF), gray matter (GM), white matter (WM), ventricular and Cerebrospinal fluid (CSF) Results Twenty one MCI and 29 NC were recruited Compared with control subjects, the elderly with MCI showed significantly decreased total intracranial volume ( P <0 05), GM volume ( P <0 01), and GM fraction ( P <0 01), and significantly increased CSF fraction ( P <0 05) The intracranial and GM volume decreased by 5 2% and 8 8% respectively, GM fraction decreased by 3 9%, and CSF fraction increased by 12 3% A linear discriminant function was obtained with stepwise discriminant analysis and variables of GM and ventricular volume entered the function ( P <0 00) with a discriminative accuracy of 74 5% Conclusion Three dimensional magnetic resonance image technique may be helpful in understanding the brain structural changes of the elderly with mild cognitive impairment
