摘 要:目的 试图通过序贯使用降钙素和甲状旁腺激素分别干预骨重建的吸收期和形成期 ,达到通过干预骨重建全过程来实现骨重建正平衡。方法 将 12只雄性大鼠平均分为对照组和干预组。干预组于实验第 1、2日每日肌肉注射益钙宁 1U/ 10 0g ,于实验的第 4、5天肌肉注射重组大鼠甲状旁腺激素 (rrPTH) 30 μg/ 10 0g ;对照组同期肌注生理盐水和溶媒。于实验第 6天取腰椎椎体 ,采用骨组织形态计量学的方法定量观察腰椎椎体松质骨骨组织学静态参数的变化 ,采用电镜定性观察各种骨重建表面上细胞的超微形态变化。结果 实验组序贯应用降钙素和甲状旁腺激素后 ,骨小梁成骨细胞表面积相对百分比、类骨质平均厚度、单位成骨细胞骨形成表面的成骨细胞数、骨小梁类骨质相对体积分别为 19 9%± 6 2 %、3 4μm± 0 4μm、37 6个 /mm± 4 6个 /mm、1 8%± 0 6 % ,均大于对照组 ,P <0 0 5 ;活性骨形成表面中骨构建形成部位的增加超过骨重建部位 ;电镜结果 :序贯应用降钙素和甲状旁腺激素后成骨细胞成骨功能增加。结论 序贯应用降钙素和甲状旁腺激素明显刺激骨形成 ,骨构建形成部位的增加大于骨重建形成部位。Objective To investigate the effect of sequential application of calcitonin and parathyroid hormone (PTH) on bone remodeling process. Methods Twelve female rats were divided equally into 2 groups: experimental group and control group. 1 U/100 g of calcitonin was injected intramuscularly to 6 rats for two days , and rrPTH in dosage of 30μg was injected intramuscularly in the fourth and fifth days. At the sixth day their lumbar vertebral bodies were resected. Undecalcified and decalcified bone sections were made to be observed by optical microscopy and electronic microscopy. Bone histomorphological parameters were measured. Normal saline and solvent of the same volume were injected intramuscularly to the controls. Results The OS/BS (%), O.Th (μm), N.Ob.S (cell/mm), and Vos/TV (%) were 19 9%±6 2%, 3 4μm ±0 4μm , 37 6 cells/mm ±4 6 cells/mm, and 1 8% ±0 6% respectively in the experimental group, significantly higher than those in the control group ( P <0 05). After the PTH administration, more and plumper osteoblasts were observed. They were rich in rough endoplasmic reticulum, mitochondria, and Golgi apparatus. Conclusion sequential application of calcitonin and parathyroid hormone obviously promotes the bone formation process. Both sites of bone remodeling formation and sites of bone modeling formation, especially the latter, increase.
