双袖套法大鼠原位肝移植术的改进

摘　　要：目的　探讨双袖套法大鼠原位肝移植模型的手术改进方法。方法　采用自制套管 ,供受体SD大鼠均取腹部横切口 ,受体无肝期前预置门静脉缝扎牵引线 ,肝上下腔静脉采用血管膜端端“三定点”吻合法 ,门静脉及肝下下腔静脉采用袖套吻合法 ,用自制“小针钩”排气。结果　行大鼠原位肝移植术 96次 ,其中正式实验 40次。改进后正式实验 ,供体手术时间 2 9min ,袖套准备时间 10min ,受体手术时间 5 1min ,无肝期 17min ;2d存活率 95 % ( 38/ 40 ) ,1周存活率 92 % ( 37/ 40 )。结论　①改进的双袖套法大鼠原位肝移植术可在单人直视下进行 ,安全确切实用。②所有吻合均应无张力、保持原位 。Objective To modify rat orthotopic liver transplantation method with two-cuff technique. Methods Some skills in Kamada's method were modified. Vein canulas were made of cotton-sticks according to vein size. Abdominal transverse incision was made both in donor and in recipient. Before the anhepatic period, tractive sutures were placed under the branch of portal vein(PV). 'Three point' anastomosis for suprahepatic vena cava(SVC) was used. PV and infrahepatic vena cava(IVC) anastomoses were made using cuff technique, while the hooked injector was used to expel air. Results Orthotopic liver transplantations were performed in 96 rats, including 56 training operations and 40 experimental operations. With the modified method, The mean time for donor operation and liver preparation were 29 min and 10 min, and the recipient operative and anhepatic time were 51 min and 17 min respectively. The two-day and one-week survival rates were 95%(38/40) and 92%(37/40). Conclusion ①This modified two-cuff method in rat orthotopic liver transplantation can simplify operation procedure, increase operative success rate. It can be performed safely, reliably and practicably by single one operator. ②All anastomosis must be kept in situ without tension. It is critical to perform the procedure carefully.
