摘 要:目的 探讨高频振荡通气在治疗新生儿肺透明膜病 (HMD)中的应用价值及其安全性。方法 分别以常频机械通气 (CMV)和高频振荡通气 (HFOV)治疗HMD患儿 ,并比较其治疗前后血气指标的变化及临床疗效。结果 1.HFOV组患儿治愈率明显高于CMV组 (6 8.0 %vs38.8% ,P <0 .0 5 ) ;2 .HFOV治疗后肺通、换气功能迅速改善 ,表现为PaCO2 迅速下降 (6 .8± 0 .4vs5 .5± 0 .6kPa ,P <0 .0 5 ) ,PaO2 明显上升 (5 .8± 1.1vs9.2± 1.7kPa,P <0 .0 5 ) ,同时所需吸入氧浓度 (FiO2 )、血氧合指数 (OI)等均迅速明显下降 (P均 <0 .0 5 ) ;3.HFOV组并发症及后遗症等均少于CMV组。结论 HFOV用于治疗HMD ,是一种疗效肯定、安全性好的新型机械通气方法。Objective To investigate and evaluate the application and safety of high frequency oscillatory ventilation in the treatment of neonates with hyaline membrane disease (HMD).Methods 43 neonates with HMD were treated by conventional mechanical ventilation (CMV) and high frequency oscillatory ventilation (HFOV), respectively. The clinical outcome and blood gas analysis were compared in two groups.Results 7 of 18 cases were cured in CMV group (38.8 %) ,17 of 25 cases in HFOV group (68.0 %) cured respectively (P<0.05). In HFOV group, more rapidly decreased PaCO 2 combined with increased PO 2 and less complications were found compared with CMV group.Conclusion Based on our information, we believed that HFOV could be more useful and safe in treatment of neonates with HMD.
