摘 要:为探讨新生儿缺氧缺血性脑损伤 (hypoxic ischemicbraindamage ,HIBD)发病机制与脑组织铁分布的关系。应用 :用 3′,3 二氨基联苯胺 ( 3′,3 diaminobezidine ,DAB)加强的普鲁士兰染色方法对尸检足月新生儿的脑皮层及皮层下白质进行铁的组织化学研究。结果显示 :1 .DAB加强的普鲁士兰铁染色切片经图像分析仪分析 ,皮层组织铁着染的面积比均大于皮层下白质 ,P值均 <0 0 1 ,HIBD组皮层及皮层下白质铁着染的面积比均大于对照组 ,P值分别为 :P <0 0 1和P <0 0 5。 2 .大脑皮层的Ⅱ、Ⅲ、Ⅳ层 ,小脑皮层的蒲肯野细胞层铁染色最明显 ,含铁细胞以神经元和胶质细胞为主 ,铁以颗粒状形式存在于细胞浆内 ,HIBD组脑组织可见更多的铁颗粒沉积。结论 :1 .足月新生儿HIBD时皮层及皮层下白质组织含铁量增多 ,脑组织含铁多的部位与HIBD易受损的部位一致 ;2 .足月新生儿HIBD易受损伤的部位与这些细胞含铁多有关。To investigate relationship of mechanism in hypoxic ischemic brain damage (HIBD) with iron distribution in brain tissues, by using A 3′,3 diaminobezidine(DAB) intensification of the Prussian Blue stain was used to examine histochemically iron in the cerebral and cerebellar cortex and subcortex white matter from 6 autopsy neonates. Results showed 1. By imaging analyzer in DAB intensification of the Prussian Blue stain slides, area ratio in the cortex elevated more apparently than that in the subcortical white matter regardless of control or HIBD group, both P <0 01. Area ratio in the cortex and subcortex white matter of HIBD group was higher than that of control group, P <0 01. and P <0 05 separately. 2. Iron granules deposited markedly in the layer Ⅱ?Ⅲ?Ⅳ of the cerebral cortex and layer Ⅱ of the cerebellar cortex. The predominant iron positive cells were neurons and glial cells. Iron granules deposited in HIBD cortex were more than that in control cortex. Conclusion. 1. Iron granules deposited in cortex tissue would increase when neonates suffered from HIBD. Region of iron deposited was just the same as that tissue susceptible to hypoxic ischemia in mature newborns. 2. The presence of iron within the cortex may be related to the elevated susceptibility of this region to oxidative damage in neonates.
