摘 要:目的 :研究胰岛素剂量和低血糖持续时间对血清酶活性影响 ,确定低血糖损伤的脏器。方法 :根据胰岛素剂量和低血糖持续时间的不同 ,将 30只家兔分为 5组 :A2 组 ,胰岛素 2U/kg低血糖持续 30min ;A10 组 ,胰岛素 10U/kg低血糖持续 30min ;B2 组 ,胰岛素 2U/kg低血糖持续 6 0min ;B10 组 :胰岛素 10U/kg低血糖持续 6 0min ;C对照组 :胰岛素 10U/kg加 5 0 %葡萄糖注射不诱发低血糖。胰岛素注射前后对血清酶的活性及肌酸激酶 (CK)同功酶的分布进行观察。结果 :所有低血糖组其血清CK的活性较对照组明显增高 ,且在A10 和B10 组CK活性的升高持续2 4h ,而血清ALT、AST、LDH的升高仅见于B10 组。此外 ,主要存在于心肌和骨骼肌中的CK Band 4在B10 组可见显著升高。结论 :低血糖所致血清酶活性及CK Band 4的升高是由于肌肉损伤的结果而非肝脏的损伤 ,低血糖持续的时间和胰岛素剂量可以影响脏器损伤的程度。Objective:To study the effects of insulin dosage and duration of hypoglycemia on serum enzyme activity in rabbits,and determine organ damage due to hypoglycemia.Method:Thirty rabbits were randomly divided into five groups according to the duration of hypoglycemia and insulin dose:A 2(hypoglycemia of 30 min duration with 2U/kg insulin),A 10 (hypoglycemia of 30 min duration with 10U/kg insulin),B 2(hypoglycemia of 60 min duration with 2U/kg insulin),B 10 (hypoglycemia of 60 min duration with 10U/kg insulin),and C(no hypoglycemia with 10 U/kg insulin and 50% glucose).Insulin induced hypoglycemia was recovered by intravenous injection of glucose.Alteration in serum enzyme activity and the distribution of creatine kinase(CK) isoenzymes were determined before and after insulin injection.Result:Serum CK activity increased significantly in all the hypoglycemic groups compared to before injection,and tended to remain high for 24h in both groups A 10 and B 10 .Serum activities of alanine aminotransferase(ALT), aspartate aminotransferase(AST) and lactate dehydrogenase(LDH) rose only in group B 10 .In addition,the level of band 4 of the serum CK isoenzymes,which exists predominantly in skeletal muscle and myocardium,increased significantly in group B 10 .Conclusion:The increase in both serum enzyme and CK band 4 isoenzyme activities during hypoglycemia is primarily due to damage in muscle rather than liver,and that the duration of hypoglycemia and insulin dosage may influence the extent of organ damage.
