摘 要:目的 改进以前的高频双向喷射通气中之反喷管较硬 ,容易损伤呼吸道 ,不利于推广应用的缺点。方法 在一类似普通气管导管的前后管壁上 ,具有 2个与导管内腔平行的管道 ,内径为 2mm ,后管的近心端约 2cm处用硅胶堵死 ,在其内壁切一小口 ,使后壁管与主导管相通 ,用于反向喷射通气。在相同条件下 ,比较高频单向喷射通气 (HFSJV)、高频双向喷射通气 (HFTJV)和HFTJV半规管组三组各通气 30min后的动脉血气值及呼气末CO2 分压 (PetCO2 )。结果 HFSJV组pH、动脉血氧分压 (PaO2 )显著或极显著低于HFTJV组和HFTJV半规管组 ,动脉血二氧化碳分压 (PaCO2 )、HCO-3 、PetCO2显著高于HFTJV组和HFTJV半规管组 ;HFTJV组PaCO2 、HCO-3 、PetCO2 均极显著或显著低于HFTJV半规管组。结论 这种新型高频双向喷射通气管 ,排除CO2 能力明显优于半规管式反喷装置及高频单向喷射通气 ,PaO2 基本稳定 ,不产生气道内负压 。Objective To improve the reverse jet ventilation tube of former high frequency two way jet ventilation (HFTJV).Methods A new HFTJV tube was set up: in the front and back tube walls of ordinary trachea tube, there were 2 pipelines with 2 mm internal diameter parallel to the inner cavity; The back tube jammed with silica gel 2 cm away from the proximal end and a hole was cut on its internal wall to communicate with the main tube; The back wall tube could provide reverse jet ventilation. On the same condition, artery gas tension and PetCO 2 in high frequency single way jet ventilation(HFSJV)、 HFTJV and HFTJV semicircular Canal Type (SCTHFTJV)30 min after ventilation were compared.Results Compared with the groups of HFTJV and SCTHFTJV, pH and PaCO 2 in the HFSJV group were decreased significantly or very significantly, PaCO 2, HCO 3, PetCO 2 increased significantly. PaCO 2, HCO 3 -, PetCO 2 in the HFTJV group were decreased significantly as compared with those in the SCTHFTJV group.Conclusion The quality of this new HFTJV tube is better than SCTHFTJV and HFSJV tube. The ability of eliminating CO 2 of HFTJV tube is obviously superior to that of SCTHFTJV and HFSJV, PaO 2 is normal, and airway pressure is not negative and cardiovascular function is normal.
