摘 要:目的 探讨咳嗽变异型哮喘(CVA)患者的小气道功能和气道反应性特点。方法 应用CSA-800肺功能测定仪和TCK-6000CV气道反应测定仪检测了44例CVA患者、17例典型哮喘患者和37例健康者的肺功能及气道反应性。结果 CVA组的一秒钟用力呼气容积与用力肺活量百分比(FEV1%)高于哮喘组,但与健康组无差异;最大呼气中段流速(MMFR)、50%肺活量最大呼气流量( 50)低于健康组,但高于哮喘组;3组间气道初始阻力(Rrs)无显著差异。CVA组的反应阈值(Dmin)高于哮喘组,其MMFR、 50、 25与Rrs呈明显负相关。结论 CVA患者存在一定程度的小气道功能障碍,气道反应阈值高于典型哮喘患者,肺功能和气道反应性特点与典型哮喘不尽相同。 Objective To explore the characteristics of small airway function and the airway reactivity in patients with cough variant asthma (CVA). Method Forty-four CVA patients, 17 typical asthma patients and 37 healthy subjects entered this study, and their pulmonary function and airway reactivity were measured. Results In CVA patients, the mean percentage of forced expiratory volume in 1 s in forced vital capacity(FEV1%) was higher than those of typical asthma patients and of healthy subjects, and no significant difference was found between the latter two. The mean maximal mid-expiratory flow rate (MMFR) and the maximum expiratory flow at 50% of vital capacity ( 50) were lower in CVA patients than in the healthy subjects, but higher than in the typical asthma patients. Respiratory resistance varied little among the 3 groups. The minimum inducing dosage (Dmin) was higher in CVA patients than in typical asthma patients. Conclusion CVA patients may have small airway function disorder, and the absence of wheezing symptoms in CVA may be due to a elevated wheezing threshold.
