摘 要:目的 :探讨海洛因中毒患者在出现神经和精神症状后的脑部MR改变。方法 :6例长期海洛因吸入患者 ,在出现共济失调、构音障碍或昏迷状态后行头部MR检查 ,对照神经解剖断面 ,明确海洛因中毒引起中枢神经系统破坏的部位、范围。结果 :双侧大脑半球病变以双侧半卵圆中心、双侧内囊后肢机率高 ,受累的机率分别为10 0 % ,但双侧半卵圆中心受累的范围不一。双侧小脑半球病变以小脑齿状核为中心 ,受累的机率为 83%。脑干病变以散在、多发和对称性为特点 ,其中黑质、红核、皮质脊髓束、内侧丘系、舌下神经核受累定位较明确。其周围的相邻结构受累 ,依据较特异性的临床表现和对照神经解剖图来推测。结论 :海洛因中毒引起的脑部改变 ,除已知的影响双侧小脑半球、双侧大脑半球脑白质及部分上下行传导束外 ,对丘脑和脑干多组神经核团的影响 ,应引起高度的重视。Objective:To investigate the cerebral MR changes after heroin poisoning. Method: 6 cases presenting ataxia,dysarthria or coma due to long term heroin inhalation underwent the brain MR examination.The location and range of the injury in central nervous system were defined. Results: In the bilateral hemispheres,the pathological change was mainly found in the bilateral hemioval areas and the posterior limbs of the bilateral inner capsule,with an incidence rate of 100%. In the bilateral cerebella,the nidus was concentrated on the dentate nuclei, with the incidence rate of 83%. In the brain stem,scattered,multiple and symmetric pathological changes were found . Conclusions: The pathological changes caused by heroin poisoning are found in white matters in the bilateral cerebrum and cerebella,and parts of the descending and ascending tracts as already well known.Besides,the thalamus and some nervous nucleus are also involved,which should be paid attention to.
