医源性胆道损伤20例诊断与治疗

摘　　要：目的　总结医源性胆道损伤的诊治经验。方法　对 2 2年间医源性胆道损伤的 2 0例患者进行回顾分析。结果　经腹胆囊切除术 (OC)损伤 6例 ,腹腔镜胆囊切除术 (LC)损伤 10例 ,胆总管探查术 (EC)损伤 4例。损害部位 :肝总管 (6例 ) ,胆总管 (9例 ) ,右肝管 (2例 ) ,左肝管 (2例 ) ,右副肝管 (1例 ) ,本组病例最终均需手术治疗 ,手术方式根据损伤部位、类型、损伤后发现的具体时间决定 ,但以规范的胆肠Roux en y吻合术疗效最满意。Objective Tosummarize the experience and lessonsdrawnfromiatrogenicbiliaryduct injury . Methods 2 0patientswithiatrogenicbiliaryductinjuriesoccurredinthepast 2 2yearswerestudied retrospectively . Results 6caseswereinjuredincholecystectomy .10casesinlaparoscopiccholecystectomy Gallbladderoperationsbyperitontoscope ,4casesinExploratoryCholecystectomy .Thelocationsofinjurieswere in 6righthepaticductcases .1casehepaticduct ,1caserightsubstantialhepaticduct.Allpatientsweretreated ultimatelybyoperations .Themethodoftreatmentwasdalliedaccordingtothetypeofinjuryandthetimeafter injury .ThemosteffectiveoperationstylewastheRoux en ycholecystectomy . Conclusion Themaincause ofiatrogenicbiliaryductinjurywaslackofresponsibilityandexperienceors killof the operator during operation .
